ÖZET
 
Amaç: Primer konjenital glokom (PKG) tanısıyla tedavi edilen olguların uzun dönem takip sonuçlarını incelemek  .
Gereç ve Yöntem: PKG tanısı ile glokom cerrahisi uygulanan 17 hastanın 32 gözünün kayıtları, retrospektif olarak incelendi.
Bulgular: Hastaların 8’i erkek 9’u kız olup, yaş ortalaması 14.35±6. 1yıl (4 ila 25 yıl) idı. İlk tanı ve cerrahi uygulama yaşı 42.64±43.7 ay (1-132 ay), bir göze uygulanan ameliyat sayısı 2.31±1.8 (1-9) idi. Olguların preoperatif göziçi basıncı 35.81±12.3 mmHg (20-59 mmHg), C/D oranı 0.72 ± 0.26 (0.1-0.9), horizontal korneal çap 12.43±1.4 mm (11-15 mm) idi. Postoperatif takip süresi 10.88±5.5 yıl olup, en son yapılan kontrolde GİB 15.41±5.1 mmHg, C/D oranı 0.67 ± 0.25 (0.2-1.0), horizontal korneal çap 13.51±1.5 mm(11-17 mm), santral kornea kalınlığı 491.4 µm(450-593 µm) idi. Buftalmus 18 gözde (% 56.25), fitizis bulbi 3 gözde (%9.37), katarak  gelişimi 8 gözde (%25)  mevcuttu. Düzeltilmiş görme keskinliği 14 gözde (% 43.75) 0.4 ve üzerinde, 16 gözde (%50) 0.1 ve altında idi. Tüm hastalarda basit miyopi ve bileşik miyopik astigmatizma saptandı. Hastaların 13’ü sosyal hayatlarını rahat sürdürebiliyorken, 4’ü yaşamlarını sürdürebilmek için yardım alıyordu.    
Sonuç: PKG olgularında GİB’nın cerrahi ve tıbbi tedavi ile kontrol altına alınması, görme prognozu açısından tek başına yeterli değildir. Aynı zamanda refraksiyon kusurunun düzeltilmesi, ambliyopinin zamanında tedavi edilmesi glokomun takip ve tedavisinde, hastaların sosyal yaşamlarını devam ettirebilmeleri için oldukça önemlidir.

Long-Term Follow up Results of Primary Congenital Glaucoma Surgery

SUMMARY
 
Purpose: To evaluate the long-term results of primary congenital glaucoma (PCG) surgery.
Material and Methods: The postoperative records of 32 eyes of 17 patients who underwent glaucoma surgery for PCG, were reviewed retrospectively.
Results: Eight patient were male, and nine patients were female. The mean age of the patients was 14.35±6.1years (4 to 25 years). The mean age at diagnosis and first  surgery was 42.64±43.7months (1-132 months), and the mean of number of surgery on the eye with congenital glaucoma was 2.31±1.8 (1-9). The mean preoperative intraocular pressure (IOP) was 35.81±12.3 mmHg (20-59 mmHg),  mean cup/disc ratio was 0.72±0.26 (0.1-0.9), mean horizontal corneal diameter was 12.43±1.4 mm (11-15 mm). The mean of postoperative follow-up was 10.88±5.5 years. At postoperative final examination, mean IOP was 15.41±5.1 mmHg, mean cup/disc ratio was  0.67±0.25 (0.2-1.0),  mean horizontal corneal diameter was 13.51±1.5 mm (11-17 mm), central corneal thickness was 491 µm (450-593 µm).  Eightteen eyes (56.25%) had buphthalmos, three eyes (9.37%) had phthisis  bulbi, eight eyes (25%) had cataract. Of eyes with succesful operations, 14 eyes (43.75%) had corrected visual acuity 0.4 or beter, 16 eyes (50%) had 0.1 or worse. All of the eyes had myopia and myopic astigmatism. Although, 13 patient can maintance their lifes without any help, but 4 patients need to help.
Conclusion: Control of IOP by medical and surgical methods is not only sufficient for good visual pognosis in primary congenital glaucoma. But also, treatment of amblyopia and to correct of refractive errors are important factors in the management of pediatric glaucoma for comfortable social life.
