ÖZET
 
Amaç: Kliniğimizin şaşılık biriminde takip edilen disosiye vertikal deviasyonlu (DVD) olguların değerlendirilmesi.
Gereç ve Yöntem: Kliniğimizin şaşılık birimine 1980-2004 yılları arasında başvuran, takip ve tedavileri yapılan ve dosya bilgilerine ulaşabildiğimiz 87 DVD’li olgu geriye dönük olarak incelenmiştir. Görme keskinliği, binoküler görme özellikleri, diğer şaşılık tipleri ve latent nistagmus ile birlikteliği, vertikal deviasyon miktarı, uygulanan cerrahi müdahale şekli kaydedilmiştir.
Bulgular: Olguların 33’ü erkek 54’ü bayan idi. Hastaların kliniğe başvuru yaşı ortalama 12.8 ± 7.6 (1-36) yıl, takip süresi ortalama 4.6±.2.5 (1-12 ) yıl idi. DVD, olguların 70’inde (%80) iki taraflı, 17’sinde (%20) tek taraflı idi. Görme keskinliği ortalama 0.8±.0.2 (0.2-1.0) düzeyindeydi. DVD en sık ezotropya ile birliktelik gösteriyordu. DVD’ye yönelik üst rektusa geriletme ve posterior fiksasyon sütürü 10 olguda iki taraflı 8 olguda tek taraflı uygulandı. Üst rektusa müdahale öncesi vertikal deviasyon miktarı 15.8±.3.3 prizma dioptri (PD) iken, operasyon sonrasında 3.3±.3.9 PD olarak saptandı.
Sonuç: DVD’de vertikal kaymaya yönelik farklı cerrahi yöntemler uygulanabilmektedir. Üst rektusa orta dereceli (4-7 mm ) geriletme ve insersiyodan 12-14 mm’ye posterior fiksasyon sütürü konulan olgularda sonuçlarımız tatmin edicidir.

The Evaluation of The Patients with Dissociated Vertical Deviation in our Clinic

SUMMARY
 
Purpose: To evaluate the characteristics of the patients with dissociated vertical deviation (DVD) followed up in our clinic.
Material and Methods: Records of 87 patients with dissociated vertical deviation who admitted to the strabismus department of our clinic between 1980-2004 were evaluated retrospectively. Visual acuity and binocular vision of patients, the association of DVD with other types of strabismus and latent nystagmus, the amount of vertical deviation and the type of the surgical approach were recorded. 
Results: Thirty three of the patients were male and 54 were female. The age of the patients at the admittance time was 12.8 ±. 7.6 (between 1-36) years and the follow-up period was 4.6±..2.5 (1-12) years.  DVD was bilateral in 70 (%80) and unilateral in 17 (%20) of the patients. Visual acuity level was 0.8±.0.2 ( 0.2-1.0). The most frequent association of DVD was with esotropia.  The recession of superior rectus muscle with posterior fixation suture for DVD was performed bilaterally in 10 and unilaterally in 8 patients. The amount of vertical deviation before and after surgery was 15.8±.3.3 prism dioptr (PD) and 3.3±.3.9 PD respectively.
Conclusion: Different surgical methods can be performed  for vertical deviation in DVD. The results of recession of superior rectus muscle 4-7 mm with posterior fixation suture to 12-14 mm from insertion of the muscle were satisfactory in our patients.
