Özet

Amaç: Tedaviye uyumun son derece önemli olduğu tüberküloz hastalarında uyumu etkileyen faktörlerle yapılan araştırma sonuçlarının profesyonel sağlık elemanlarının yapabilecekleri girişimlere ışık tutacağına inanılmaktadır. Bu amaçla, araştırma tüberküloz hastalarının ilaç tedavisine uyumunu ve uyumu etkileyen faktörleri belirlemek amacıyla tanımlayıcı olarak yapılmıştır.
Gereç ve Yöntem: Araştırma Erzurum Verem Savaş Dispanserin’de tedavi gören hastalar üzerinde yapılmıştır. Veriler 05.04.2004-05-06-2004 tarihleri arasında 2003 yılında tüberküloz tanısı almış ve Verem Savaş Dispanserinde kayıtlı olan 116 tüberküloz hastasından elde edilmiştir. Veriler literatür ışığında hazırlanmış olan, hastaya ait tanıtıcı özellikler ile tedaviye uyumu belirlemeye yönelik soruları içeren anket formu ile toplanmıştır. Veriler; yüzdelik oranlar ve Ki kare testleriyle değerlendirilmiştir.
Bulgular: Hastaların %66.4’ünün hastane deneyimi olduğu, %65.5’inin önerilen kontrollere gitmediği, sağlık çalışanlarının önerilerine karşı hastaların %48.3’ünün “ne söylenirse yaparım” dediği tespit edilmiştir. Tedaviye uyum durumunu belirlemeye yönelik; hastaların %42.2’sinin ilacı önerilen zamanda kullandığı, %24.1’inin ilacını uygun dozda kullandığı, %82.8’inin de ilacın kullanım şeklini bildiği saptanmıştır. Çalışmamızın bir sonucu olarak hastaların % 57.8’inin tanımlanan ilaçları önerilen zamanda almadığı, % 75.9’unun tanımlanan ilaçları önerilen dozda almadığı ve %82.8’ inin ilaçların kullanım şeklini bildiği bulunmuştur. 
Sonuç: Hastaların ilaçlarına uyumunu arttırmada hasta aileleri ve yakınları ile işbirliği kurulması ve onların da bu konuda eğitilmesi, ilaç uyumuna etki edebilecek olan bağımsız değişkenlerin sonuçlara etkisini ortaya koymak amacı ile deneysel araştırmaların planlanması, yapılması ayrıca hastaların ilaçlarına uyumunun maliyet, kar-zarar analizinin yapılması önerilmektedir.

Compliance to Treatment of The Patients with Tuberculosis and Determination of The Factors Affecting

Summary

Aim:  The research result related to the compliance in the patients with tuberculosis in whom compliance with the medication is very important is believed to enlighten the attempts of Professional health staff. This study was carried out descriptively with the aim of determining the factors affecting the compliance and the adaptation to the treatment of the patients with tuberculosis. 
Material and Methods: This study was carried out on the patients who took treatment in Tuberculosis Struggle Out-patient Clinic. Data were collected from 116 tuberculosis patient who diagnosed in 2003 and registered in Tuberculosis Struggle Out-patient Clinic between the dates 5th April 2004 and 5th June 2004 in Erzurum.
The data were collected by means of questionnaire form prepared in the light of literature and including questions dealing with the adaptation to the treatment and informative features about the patient. The data were evaluated by means of percentage – proportions and chi-square tests.
Results: It was detected that 66.4% of the patients had hospital experience, and that 66.5% of them didn’t go to recommended controls, and that 48.3% of the patients had said that “no matter what the health staff said, I perform“against the recommendation of health staff”.
In direction to the determination of treatment adaptation: It was detected that 42.2% of the patients used the medicine at recommended time, and that 24.1% of the patients used the medicine in suitable dose, and that 82.8% of the patient knew how to use the medicine.
As a result of our research it was found out that 57.8% of the patients do not take the prescribed drugs at the recommended time, 75.9% of them do not take the prescribed drugs at the recommended dosage and 82.8% know the way to use the drugs.
Conclusion: To increase the adaptation to the medicine of the patients, it has been suggested that close cooperation should be realized with patients’ families and relatives, and that they should be educated about this matter, and that experimental researches should be planned and done with the aim of bearing out the effect of independent variances to have effect on the medicine adaptation to the results, besides, it is suggested that cost, profit and loss analysis of compliance with the medication of the patients should be done.
