Retina Distrofilerinde Fundus Otoflöresans Özellikleri Elçin SÜREN, Figen BATIOĞLU, Emin ÖZMERT ÖZET Amaç: Retina distrofilerinde klinik muayene ve fundus flöresein anjiografi bulgularının yanı sıra fundus otoflöresans özelliklerinin belirlenmesi Gereç ve Yöntem: Farklı tiplerde retina distrofisi olan 27 olgunun 54 gözü çalışma kapsamına alındı. Olguların 8’i Stargardt maküler distrofi / fundus flavimakulatus (16 göz), 11’i retinitis pigmentoza (22 göz), 1’i öküz gözü makülopati (2 göz), 5’i Best vitelliform maküler distrofi (10 göz) ve 2’si erişkin tip vitelliform maküler distrofi (4 göz) idi. Olguların tam oftalmolojik muayenelerini takiben renkli fundus fotoğrafları çekildi ve flöresein anjiografi uygulandı. Fundus otoflöresans (FAF) görüntüleri Heidelberg Retinal Anjiografi 2 (HRA2) kullanılarak kaydedildi. Tüm görüntülemelerden elde edilen bulgular değerlendirildi. Bulgular: Stargardt ve fundus flavimakulatusta merkezde oval hipootoflöresansla birlikte çevresinde düzensiz flöresans paterni mevcuttu. Retinitis pigmentoza’da perifoveal halka şeklinde hiperotoflöresans izlenirken, erişkin tip vitelliform distrofili hastalarda vitelliform lezyonlar artmış otoflöresans göstermekte ve atrofik evrede de bu özelliğini devam ettirmekteydi. Best distrofide vitelliform lezyon artmış otoflöresans patern göstermekteydi. Öküz gözü makülopatide ise merkezde hipootoflöresans, çevresindeki retina pigment epitel (RPE) atrofisi alanlarında ise hipootoflöresans ağırlıklı düzensiz otoflöresans paterni izlendi. Sonuç: Retinanın distrofik hastalıklarında, lipofuskin dağılımı ve RPE’nin işlevsel bozuklukları ile ilişkili olarak farklı otoflöresans paternleri ortaya çıkmaktadır. Fundus otoflöresans görüntüleme bu hastalıklarda, muayene ve flöresein anjiografi bulgularına ilave bilgiler sağlayan yararlı ve invaziv olmayan bir tekniktir. Anahtar Kelimeler: Best vitelliform maküler distrofi, Erişkin tip vitelliform distrofi, Fundus flavimakulatus, Fundus otoflöresans, Retinitis pigmentoza, Stargardt maküla distrofisi Fundus Autofluorescence Imaging in Retinal Dystrophies SUMMARY Aim: To evaluate autofluoresence patterns of retinal dystrophies in addition to clinical and fluoresein angiography findings. Material and method: Fifty-four eyes of twenty seven patients with different types of retinal dystrophies were included. Of the patients, 8 had Stargardt macular dystrophy / fundus flavimaculatus (16 eyes), 11 had retinitis pigmentosa (22 eyes), 1 had bull’s eye maculopathy (2 eyes), 5 had Best vitelliform macular dystrophy (10 eyes) and 2 had adult vitelliform dystrophy (4 eyes). After complete ophthalmologic examination, color fundus photographs were taken and fluorescein angiography was performed. Fundus autofluorosence (FAF) images were obtained by Heidelberg Retinal Angiography 2 (HRA2). All images were analysed. Results: Oval area of hypoautoflouresence and irregular fluoresence patterns arround this area were typical in cases with Stargardt disease and fundus flavimaculatus. Hyperautofluorescent perifoveal ring was observed in cases with retinitis pigmentosa. In adult vitelliform dystrophy, vitelliform lesions showed increased FAF pattern which was observed also at atrophic stages. In Best patients, vitelliform lesions were hyperautofluorescent. Central hypoautofluoresencent area and irregular AF patterns were observed in eyes with bull’s eye maculopathy. Conclusion: Lipofuscin accumulation in RPE and RPE functions were associated with various patterns of fundus autofluoresence in retinal dystrophies. Fundus autofluoresence imaging is a useful and noninvasive technique for diagnosis and follow up for such diseases when it is combined with clinic and fluorescein angiography.
