Pediatrik ve Afakik Kontakt Lens Uygulamaları Güncel Durum Meltem YAĞMUR ÖZET Pediatrik katarakt olgularında, cerrahiyi takiben en kısa sürede hastalara kontakt lens verilmelidir. Pediatrik afakide yumuşak, rijid gaz geçirgen ya da silikon kontakt lenslerle düzeltme yapılabilir. Günümüzde bu hastalarda çok yüksek oksijen geçirgenliğine sahip olan silikon lensler uzun süreli kullanımda tercih edilmektedir. Silikon lenslerin daha dik temel eğriliği, küçük çapı ve yüksek dioptrideki lens gücü gibi parametreleri ile pediatrik afak hastalardaki beklentileri karşılamakta ve diğer lenslere üstünlük sağlamaktadır. Silikon kontakt lenslerin en önemli dezavantajları, yüzey ıslanabilirliğinin az olması, mukusun kolaylıkla lense yapışması ve protein birikiminin olması ve yüksek maliyetidir. Pediatrik kontakt lens uygulamasında aile; kontakt lenslerle ortaya çıkabilecek sorunlar, kontakt lenslerin çıkarılıp takılması ve lens bakım sistemlerinin uygulanması konularında bilgilendirilmelidir. Kontakt lensler ile birlikte tercih edilen göze kapama tedavisinin başlanması ambliyopinin önlenmesinde çok önemlidir. Anahtar Kelimeler: Pediatrik afaki, Silikon kontakt lens, Ambliyopi The Use of Contact Lenses for Pediatric Aphakia - Current Situation SUMMARY Following the surgery of pediatric cataract cases, patients should be given contact lenses at the earliest possible time. Optical correction of pediatric aphakia can be observed with the use of soft, rigid gas permeable or silicone lenses. Nowadays, these patients can also benefit from the use of highest oxygen permeable silicone contact lenses for extended wear. Silicone lenses, as opposed to other lenses, give better results with the treatment of steeper base curve, small diameter and high plus refractive power, in pediatric aphakia cases. Some of the main disadvantages with silicon contact lenses are, poor lens surface wetting, the adhesion of mucous to the lens, the accumulation of the protein and the high cost of the lenses. Information on the problems arising with the use of lenses, the insertion and removal of lenses and the care system of pediatric contact lenses, should be given to the families. Contact lenses together with the treatment of occlusion of the preferred eye, are very important in the prevention of amblyopia. Key Words: Pediatric aphakia, Silicone contact lens, Amblyopia
