Ömür UÇAKHAN GÜNDÜZ ÖZET Keratokonusta görsel rehabilitasyon büyük oranda sert kontakt lenslerle sağlanabilirken bazı seçilmiş olgularda alternatif yöntemlerden faydalanılabilir. Bu yöntemlerden ikisi intrastromal halka segmentler (INTACS) ve kollajen çapraz bağlanmadır. INTACS, santral korneası saydam olup kontakt lens intoleransı olan veya lens uymayan keratokonus olgularında görmeyi ve/veya kontakt lens toleransını arttırmak, keratoplastiyi geciktirmek veya gerekliliğini ortadan kaldırmak amacıyla uygulanan bir yöntemdir. Korneal çapraz bağlanma ise riboflavin ve ultraviyole-A ışığı kullanılarak kornea kollajen molekülleri arasında ek kovalen bağlanma oluşturulması tekniğidir. Bu sayede, kollajen çatının stabilitesi ve kornea sertliği artırılarak, ilerleyici kornea incelmesinin, dolayısıyla keratokonus progresyonunun önlenmesi amaçlanmaktadır. Anahtar Kelimeler: Keratokonus, İntrakorneal halkalar, Riboflavin ile kollajen çapraz bağlanma Alternative Treatment Modalities in Keratoconus: Intrastromal Corneal Ring Segments and Collagen Crosslinking (CXL): Update SUMMARY While visual rehabilitation is achieved in most part by rigid contact lenses in keratoconus, in selected subjects other treatment modalities may be utilized. Among such methods are the intrastromal corneal ring segments (INTACS) and collagen cross linking. INTACS are implanted in eyes of patients with clear central cornea and contact lens intolerance or in whom an adequate fit could not be achieved with the purpose of increasing the visual acuity and/or improving contact lens tolerance, postphonening or eliminating the need for penetrating keratoplasty. Collagen cross linking (CXL), on the other hand, involves usage of riboflavin and ultraviolet-A light to establish additional covalent binding between corneal collagen molecules. The technique stabilizes the collagen scaffold, increases corneal stiffness and counteracts progressive corneal thinning, thus, is hoped to halt the progression of keratoconus.
