Keratokonusta Rose K2 Kontakt Lens Uygulanımı Elçin SÜREN, Ömür Ö. UÇAKHAN GÜNDÜZ, Ayfer KANPOLAT ÖZET Amaç: Farklı derecelerdeki keratokonuslu gözlerde Rose K2 kontakt lens uygulama sonuçlarının değerlendirilmesi. Gereç ve yöntem: Rose K2 kontakt lensleri uygulanan keratokonus hastaları retrospektif olarak incelendi. Olgular uygulama öncesinde tam oftalmolojik muayene ile değerlendirildi ve keratometri değerlerine göre hafif, orta, ileri ve çok ileri keratokonus olmak üzere 4 gruba ayrıldı. Takip sonunda olgular lens uyumu ve görme rehabilitasyonu açısından değerlendirildi. Bulgular: Yüzbeş hastanın 183 gözü çalışma kapsamına alındı. Yirmiüç gözde (%12,5) hafif, 96 gözde (%52,5) orta, 51 gözde (%27,8) ileri, 13 gözde (%7,1) çok ileri keratokonus tespit edildi. Kontakt lens uygulaması sonrası ortalama takip süresi 14±1,5 aydı (3 ay-36 ay). Ortalama düzeltilmemiş, gözlükle en iyi düzeltilmiş ve son kontrolde kontakt lens ile en iyi düzeltilmiş görme keskinlikleri sırasıyla hafif keratokonusta 0,59±0,30, 0,16±0,17, 0,004±0,20, orta keratokonusta 0,76±0,39, 0,24±0,25, 0,01±0,05, ileri keratokonusta 1,19±0,27, 0,71±0,38 , 0,07±0,12, çok ileri keratokonusta ise 1,56±0,28, 1,27±0,41, 0,16±0,12 logMar olarak saptandı. Tüm olgularda yeterli kontakt lens uyumu elde edildi ve sonuç lens ile en iyi görme keskinliğine ulaşıldı. Ortalama düzeltilmemiş görme keskinlikleri, gözlükle ve kontakt lens ile en iyi düzeltilmiş görme keskinlikleri arasında istatistiksel olarak anlamlı farklar saptandı (p<0,05). Sonuç lensin her keratokonus evresinde, ortalama keratometri değerine göre % 63,4-%100 oranla daha düz eğimde kesinleştirildiği görüldü. Sonuç: Rose K2 kontakt lensleri keratokonusun ileri evreleri de dahil olmak üzere tüm evrelerde başarıyla uygulanabilmektedir. İlk deneme lensinin doğru seçimi deneme süresini kısaltabilir. Evre ilerledikçe, kornea ortalama eğimine göre daha düz eğimde sonuç lens uygun olmaktadır. Anahtar Kelimeler: Keratokonus, Kontakt lens, Rose K2 Rose K2 Contact Lens Fitting in Keratoconus SUMMARY Aim: To evaluate the results of Rose K2 contact lenses in different grades of keratoconus. Material and Method: Keratoconus patients to be fit with the Rose K2 contact lenses were analysed retrospectively. Prior to lens fit, all patients underwent a complete ophthalmologic examination and were divided into 4 groups as mild, moderate, severe and advanced keratoconus according to the keratometric values. Patients were evaluated with regards to the lens fitting characteristics and visual rehabilitation at the end of the follow up period. Results: One hundred eighty three eyes of 105 patients were included in the study. Twenty three eyes (12.5%) had mild, 96 eyes (52.5%) moderate, 51 eyes (27.86%) severe and 13 eyes (7.1%) advanced keratoconus. The mean follow up period was 14±1.5 months (3 -36 months) after contact lens fit. The mean uncorrected, best spectacle corrected and the best contact lens corrected visual acuities were 0.59±0.30, 0.16±0.17, 0.004±0.2 in mild, 0.76±0.39, 0.24±0.25, 0.01±0.05 in moderate, 1.19±0.27, 0.71±0.38, 0.07±0.12 in severe and 1.56±0.28, 1.27±0.41, 0.16±0.12 logMar in advanced keratoconus, respectively. In all patients, adequate contact lens fit was obtained and best corrected visual acuity was achieved with the final lens. There were significant differences between the mean uncorrected as well as best spectacle and contact lens corrected visual acuities (p<0.05). Final lens was fit 63.4%-100% flatter than the mean keratometric value at each stage of keratoconus. Conclusion: Rose K2 contact lenses may be successfully fit in keratoconic eyes regardless of the keratoconus grade. Proper selection of the first trial lens may shorten the trial period. As the grade of keratoconus increases, the trial lens is fit flatter than the mean corneal keratometry. Key Words: Keratoconus, Contact lens, Rose K2
