Tam Tendon Vertikal Kas Tranpozisyon Cerrahisi ile Birlikte Posterior Fiksasyon: Üç Olguda Klinik Deneyim Sılay CANTÜRK UĞURBAŞ, Sibel OTO, Yonca AYDIN AKOVA ÖZET Tam tendon vertikal kas transpozisyonu ve posterior fiksasyon sütürü uyguladığımız iki altıncı sinir paralizisi ve bir Duane sendromu olan üç olgu değerlendirildi. Preoperatif kayma açısı primer pozisyonda 33± 7 PD (prizma diyoptri; 25-40 ET) iken postoperatif dönemde 3 ± 8 PD’ye (6 XT– 10 ET) geriledi. Baş pozisyonu düzeldi. Abdüksiyon kısıtlılığı -6 ve -4 ten - 2 ye geriledi. Duane sendromu olan hastaya iki aşamalı cerrahi uygulandı. Abdüksiyon kısıtlılığı -4’ten -3 ‘e geriledi. Posterior fiksasyon sütürü ile birlikte tam tendon vertikal kas cerrahisi pasif abdüksiyon etkisini artırabildiği ve lateral rektus fonksiyonunun tamamen yok olduğu altıncı sinir olgularında etkili olduğu görülmüştür. Duane sendromu olan olgularda bu tekniğin abdüksiyona etkisi iç rektus restriksiyonu ile sınırlı olduğu düşünülmektedir. Anahtar Kelimeler: Altıncı sinir paralizisi, Duane sendromu, Transpozisyon cerrahisi, Posterior fiksasyon sütürü Full Tendon Vertical Rectus Transposition Surgery with Posterior Augmentation: Report of Three Cases SUMMARY Two patients with chronic sixth nerve palsy and one patient with Duane syndrome underwent full tendon vertical rectus transposition with posterior suture fixation. Postoperative change in alignment at primary gaze, limitation of abduction, improved ocular motility and resolution of face turn were evaluated. Preoperative deviation at primary position was 33± 7 PD (prism diopter; 25-40 ET) and improved to 3 ± 8 PD ( 6 XT-10 ET). Head posture resolved in after surgery. Abduction deficiency improved from -6 and -4 to -2. Duane patient underwent two staged surgery. Face tilt resolved and limitation of abduction improved from -4 to -3. Full tendon transposition with posterior fixation suture has enabled satisfactory alignment and resolution of face turn in two cases of long standing sixth nerve palsy. The response to the surgery in Type I Duane patients with esotropia is thought to be limited by the existing amount of medial rectus muscle restriction.
