Özet

Tiroid fonksiyon bozukluklarının bir çok nöromüsküler bozukluğa yol açtığı bilinmektedir. Bununla beraber izole nöromüsküler bulgularla ortaya çıkan tiroid fonksiyon bozuklukları oldukça nadir görülmektedir. Biz bu bildiride statin tedavisinin başlanmasından sonra miyopati bulguları ile nöroloji polikliniğine başvurmuş olan bir hipotroidi olgusunu sunuyoruz.
Yorgunluk kas ağrıları, kas krampları ve uyuşukluk yakınmaları ile polikliniğimize başvuran 67 yaşındaki erkek hastanın bu yakınmalarının yaklaşık 3 ay önce başladığı öğrenildi. Ancak 1 ay önce hiperkolesterolemi nedeniyle başlanmış olan statin tedavisi ile yakınmalarının şiddeti belirgin şekilde artmış. Öz ve soy geçmişinde özellik saptanmadı. Fizik muayene bulguları pretibial ödem dışında normal bulundu. Nörolojik muayenesinde derin tendon reflekslerinin hipoaktif olduğu ve kas güçlerinin azaldığı görüldü. Hastanın kan tetkiklerinde TSH: 100 (0,27-0,42) uIu/ml, serbest T3: 0,26 (2-4,4) pg/dl, serbest T4:0,081 (0,93-1,17) ng/dl ve kreatin kinaz :4988 U/L (0-200) bulunurken ENMG incelemelerinde miyopati ve nöropati ile uyumlu bulgular saptandı. Statin tedavisi kesilerek tİroid hormon replasman tedavisi başlanan hastanın klinik bulgularının hızla düzeldiği izlendi.
Statin tedavisi planlanan hastalarda; tiroid fonksiyon bozukluklarının da hiperkolesterolemi yapabildiği ve statin tedavisi ile tiroid bozukluğunun yol açtığı miyopati tablosunun daha da kötüleşeceği hatırlanmalıdır.

A Statin Induced Myopathy in a Patient with Undiagnosed Hypothyroidism

Summary

It is well known that thyroid diseases may result in a variety of neuromuscular disturbances. However it is rare to see thyroid disoders presenting with isolated neuromuscular disturbances. Here we present a patient with hypothyroidism attended to our neurology out-patient clinic with myopathy signs one month after the statin treatment.
67 years old male patient had a history of malaise, muscle pains, cramps and numbness for last three monts. His symptoms worsened after the statin treatment was started for hypercholestorelemia one month ago. His personal or family history revealed no significance. His physical examination was normal except pretibial edema. On neurologic examination, deep tendon reflexes were hypoactive and the muscle strength was decreased. Laboratory result were as following; TSH: 100 (0,27-0,42) uIu/ml, free T3: 0,26 (2-4,4) pg/dl, free T4:0,081 (0,93-1,17) ng/dl ve creatine kinase: 4988 U/L (0- 200) and the ENMG results were consistent with myopathy and neuropathy. The symptoms of the patient improved rapidly after the cessation of station and onset of thyroid replacement therapy.
Whenever the statin treatment is planned, it should be kept in mind that thyroid function abnormalities also can result in hypercholesterolemia and the thyroid induced myopathy symptoms may further worsen after the statin is started.
