Bilateral Optik Nörit ile Ortaya Çıkan Behçet Hastalığı Olgusu Ali AYDIN, Özlem KARABUDAK, Hakan TEREKECİ, Burhan YILMAZ, Dilaver ERŞANLI ÖZET Bu çalışmada bilateral optik nörit saptanan, etiyolojik araştırmada Behçet Hastalığı ortaya çıkarılan bir olguyu sunduk. Birkaç gün içinde gelişen baş ağrısı ve her iki gözde görme bulanıklığı ile servisimize başvuran 28 yaşında bir erkek hastada görme keskinliği sağda 6/10, solda 7/10 düzeyindeydi. Ön segment muayenesi normaldi. Fundus muayenesinde her iki göz optik disk sınırları silikleşmiş, disk kabarık bir görünüm sergilemekteydi. Retinal venlerde disk çevresinde minimal tortiozite izlendi. Fundus floresein anjiyografide her iki gözde optik diskte 2. dakikadan itibaren floresein sızdırması görüldü. SITA standart 30-2 görme alanı testinde her iki gözde konsantrik daralma saptandı. Etyolojik araştırmalarda yaklaşık altı aydır ağızda ve skrotumda tekrarlayan ülserler, ekstremitelerde ciltte eritema nodozum benzeri lezyonlarının olduğu ancak hastanın bunları önemsemediği anlaşıldı. HLA-B51 pozitif olan hastanın Dermatoloji ve İç hastalıkları konsültasyonları sonucu Behçet hastalığı tanısı konulan hastada nörolojik araştırmalarda nöro-Behçet bulgusu saptanmadı. Hastaya sistemik steroid tedavisi uygulanarak optik nöritte tam düzelme sağlandı. Sonuçta izole optik nörit Behçet hastalığının çok az rastlanan bir göz bulgusudur. Optik nörit sonrası Behçet Hastalığı tanısı konulan olgular daha da nadirdir. Herhangi bir üveit bulgusu olmaksızın bilateral optik nörit saptanan bu Behçet hastalığı olgusu bu nedenle sunuldu. Anahtar Kelimeler: Behçet Hastalığı, Nöro-Behçet, Optik nörit A Case of Behçet’s Disease Presenting with Bilateral Optic Neuritis SUMMARY In this study we reported a case with bilateral optic neuritis, in that Behçet disease was diagnosed as the etiological problem. A 28-year-old man presented with headache and bilaterally blurred vision started a few days ago. Visual acuity was 6/10 in the right and 7/10 in the left eye. Anterior segment examination was normal. Ophthalmoscopy revealed bilateral swelling of optic nerve heads and minimal tortuosity of retinal veins around the disc. Fundus fluorescein angiography highlighted florescein leakage of disc bilaterally since 2nd minute. Visual field constriction was present in the SITA standard 30-2 testing of two eyes. In etiologic investigation, it was revealed that the patient had had recurrent oral and scrotal ulcers and some dermal lesions resembling erythema nodosum for 6 months but he did not mind about them. HLA-B51 was positive. Based on the consultations of Dermatology and Internal medicine, the patient was diagnosed as Behçet’s disease. Any finding of neuro-Behçet was found in neurological examination. Bilateral optic neuritis was recovered following systemic steroid therapy. In conclusion, isolated optic neuritis is a rare ocular finding of Behçet’s disease. The cases with optic neuritis as presenting finding are extremely rare. So, we decided to report this infrequent case with bilateral optic neuritis as presenting symptom of Behçet’s disease. Key Words: Behçet’s disease, Neuro-Behçet, Optic neuritis
