Eksternal Dakriyosistorinostomi Cerrahisinde Tek Flep ve Çift Flep Sonuçlarının Karşılaştırılması Mehmet Kaan KAYA, Tamer DEMİR Özet Amaç: Eksternal dakriyosistorinostomi cerrahisinde tek flep ve çift flep sonuçlarını karşılaştırmak. Gereç ve Yöntem: Fırat Üniversitesi Hastanesi Göz Kliniği’nde Nisan 2005-Aralık 2009 tarihleri arasında eksternal dakriyosistorinostomi uygulanan 114 hastanın 114 gözü retrospektif olarak değerlendirildi. Bulgular: Çift flep DSR uygulanan 44 gözün 2’sinde nüks izlendi. Çift flep DSR de başarı oranı %95,45 olarak tespit edildi. Tek flep DSR uygulanan 70 gözün 3’ünde nüks izlendi. Tek flep DSR de başarı oranı % 95,71 olarak belirlendi. Her iki grupta nüks izlenen hastalara revizyon DSR yapıldı. Çift flep DSR uygulanan 1 hastada postoperatif hemoraji izlendi. İki günlük burun tamponadı ile sorun giderildi. Bir olguda tüpe bağlı punktal erezyon görüldüğü için silikon tüp 3. ayda çıkartıldı. Bir olguda silikon tüpe bağlı postoperatif 2. ayda punktal granülasyon gelişti. Sonuç: Çalışmamız eksternal dakriyosistorinostomi cerrahisinde cerrahi başarı açısından çift flep anastomozlu DSR’nin, tek flep anastomozlu DSR’den farklı olmadığını göstermektedir. Yalnızca ön fleplerin sütüre edildiği operasyon hem kolay ve hızlı hem de çift flep tekniği ile benzer başarı oranlarına sahiptir. Anahtar Kelimeler: Eksternal dakriyosistorinostomi, Tek flep, Çift flep Comparison of Surgical Outcomes of External Dacryocystorhinostomy with Single Flap or Double Flaps SUMMARY Aim: To evaluate the surgical outcomes of external dacryocystorhinostomy with single flap or double flaps. Material and Method: Retrospective data of 114 eyes of 114 patients undergoing external dacryocystorhinostomy between April 2005-December 2009 in Firat University Hospital Eye Clinic is evaluated. Results: We determined 2 recurrences in 44 eyes which were performed double flaps. The success rate was %95.45 in the double flaps group. We determined 3 recurrences in 70 eyes which were performed single flap group. The success rate was %95.71 in the single flap group. The patients with recurrences in both groups were performed revised DSR. Postoperative hemorrhage was seen in only 1 patient in the double flaps group. This problem was solved with nasal tamponade for two days. In 1 eye punctum erosion was observed due to the irritation of the silicon tube and it was removed 3 months after surgery. In one eye punctum granulation was developed 2 months after surgery. Conclusion: Our study suggests that dacryocystorhinostomy with double-flaps anastomosis has no advantage over dacryocystorhinostomy with only single flap. Anastomosis by suturing only anterior flaps is easier and faster to perform and the success rates are similar to double flaps technique.
