Fibrin Yapıştırıcı ve Konjunktival Otogreft ile Yapılan Pterjiyum Cerrahisinin Uzun Dönem Sonuçları Ertuğrul CAN, Sefer Ogün BÖLÜK Özet Amaç: Konjunktival otogreft ve fibrin yapıştırıcı kullanarak yapılan pterjiyum cerrahisinde etkinliği, güvenirliği, nüks oranını ve aynı zamanda ameliyat esnasında ve sonrasında komplikasyonları değerlendirmek. Gereç ve Yöntem: Primer pterjiyum nedeniyle ameliyat edilen 605 hastanın 632 gözü retrospektif olarak incelendi. Nüks oranı, ameliyat esnasında ve sonrasında oluşan komplikasyonlar değerlendirildi. Hastaların hepsinde aynı gözden alınan konjunktiva otogrefti ve fibrin yapıştırıcı kullanıldı. Hastaların hepsinin 18-30 ay boyunca muayene sonuçları takip edildi. Bulgular: Hastaların ortalama yaş ortalaması 53,4 (38-81) yıl idi. Bunların 282’si (%46,6) kadın ve 323’ü (%53,4) erkek hasta idi. Pterjiyum nüksü 21 (%3,3) hastada gözlendi. Ameliyat sonrası 7 (%1,1) gözde greft kaybı, 181 (%28,6) gözde yalancı ödem, 195 (%30) gözde hafif greft retraksiyonu ve 4 gözde granülom oluşumu görüldü. Ameliyat esnasında hiçbir komplikasyon görülmedi. Sonuç: Pterjiyum cerrahisinde konjunktival otogreftin fibrin yapıştırıcı ile birlikte kullanılması güvenli ve etkili bir yöntemdir ve nüks oranı düşüktür. Fibrin yapıştırıcın kullanılması ayrıca sütüre bağlı komplikasyonların oluşmasını ve yabancı cisim hissini azaltmaktadır. Anahtar Kelimeler: Fibrin yapıştırıcı, Konjunktiva otogrefti, Pterjiyum cerrahisi Long Term Results of Pterygium Surgery Using a Fibrin Adhesive and Conjunctival Autograft SUMMARY Aim: To evaluate the efficacy and safety of fibrin glue to attach conjunctival autografts as well as recurrence rate, intraoperative and postoperative complications in pterygium surgery. Material and Method: Six hundred and thirty-two eyes of 605 patients operated for primary pterygium were evaluated retrospectively for recurrence rate and intraoperative and postoperative complications. All patients were operated using an ipsilateral conjunctival autograft fixed with fibrin glue. Patients were followed up for 18 to 30 months after surgery. Results: Mean age was 53,4 years (range 38-81). There were 282 (46.6%) women and 323 (53.4%) men. Pterygium recurrence was observed in 21 (3.3%) eyes. Other postsurgical complications were lost of graft in 7 (1.1%) eyes, graft pseudo edema in 181 (28.6%) eyes, slight graft retraction in 195 (30%) eyes and donor site granuloma in 4 (0.6%) eyes. No intraoperative complications were noted. Conclusion: The use of fibrin glue seems to be a safe, easy and efficacious method for attaching the conjunctival autograft in pterygium surgery and associated with a low rate of recurrence. The use of fibrin glue also avoids complications due to sutures and reduces the foreign body sensation. Key Words: Conjunctival autograft, Fibrin glue, Pterygium surgery
