Anizometropik Ambliyopili Olgularda Kırma Kusurlarındaki Değişim ve Kapama Tedavisinin Etkinliği H. Ekrem KAYA, Huban ATİLLA Özet Amaç: Anizometropik ambliyopisi olan çocuklarda, görsel gelişim sürecinde verilen gözlük ve kapama tedavisinin etkinliğini belirlemek ve kırma kusuru üzerindeki etkisini araştırmak. Gereç ve Yöntem: Retrospektif olarak dosya tarama yöntemi ile 5 ile 8 yaşları arasında kliniğimizde görülerek ilk kez tanı alan ve tedavi verilerek en az 3 yıl takibi yapılan 132 olgu çalışma kapsamına alındı. Her hastada göz muayene bulguları kaydedildi ve uygulanan tedaviye göre gözlük kullanan ve gözlükle birlikte kapama yapanlar olarak iki gruba ayrıldı. Bulgular: Her iki grupta da 3 yıllık takip sonunda ambliyop olan gözde görme keskinliğinde anlamlı artış tespit edildi ancak kapama yapan grupta artış daha fazlaydı. Sadece gözlük kullanan grupta başlangıç muayenesine göre sferik retinoskopi değerinde -0,71 D’lik değişim saptanırken, gözlükle birlikte kapama yapan gruptaki olgularda ise -0,53 D’lik azalma saptandı fakat bu fark istatistiksel olarak anlamlı değildi. Üç yıllık takip sonunda astigmatizma ve anizometropi değişim değerlerinde iki grup arasında istatistiksel olarak anlamlı fark izlenmemiştir. Sonuç: Anizometropik ambliyopili olgularda gözlük ve birlikte yapılan kapama tedavisi görme keskinliğinde artış sağlamakta ancak kırma kusurunda yaş ile birlikte olan değişimi etkilememektedir. Anahtar Kelimeler: Ambliyopi, Kapama tedavisi, Kırma kusurları Refractive Error Changes in Subjects with Anisometropic Amblyopia and Efficacy of Occlusion Therapy SUMMARY Aim: To investigate the effects of spectacle use and patching treatment on refractive error changes during visual development period in anisometropic amblyopic children and to determine the efficacy of patching treatment. Material and Method: Retrospectively, the records of patients that were diagnosed as anisometropic amblyopes were reviewed and 132 subjects between the ages of 5 to 8 at the time of diagnosis, with minimum follow-up of 3 years were included in the study. Full ophthalmological examination data from every visit was retrieved for evaluation. Patients were divided into 2 groups according to the treatment they applied; spectacle alone and spectacle plus patching groups. Results: Increase in visual acuity was detected in the amblyopic eyes in both groups after 3 years follow-up however the increase was higher in the spectacle plus patching group. In spectacle group there was a -0.71 D change from the baseline spheric retinoscopy measurement and in spectacle plus patching group there was -0.53 D decrease but the difference was not statistically significant. In 3 years follow-up period, astigmatism and anisometropia scores did not show any statistically significant difference between 2 groups. Conclusion: In anisometropic amblyopia cases, spectacles with patching treatment increases the visual acuity however does not change the anisometropia score and the changes in refractive errors are similar to the only spectacle using children.
