Seronegatif ve Seropozitif Romatolojik Hastalıklarda Santral ve Periferik Kornea Kalınlığı Yelda YILDIZ TAŞÇI, Özge SARAÇ, Canan GÜRDAL, Şükran ERTEN, Serdar BAYRAKTAR ÖZET Amaç: Bu çalışmanın amacı seronegatif (romatoid faktör (RF) negatif) ve seropozitif (RF pozitif) romatolojik hastalıklarda santral ve periferik kornea kalınlığının ön segment optik koherans tomografisi (OKT) ile değerlendirilmesi ve bu hastalıklarda kornea kalınlığına RF’in etkisinin araştırılmasıdır. Gereç ve Yöntem: Seronegatif romatoid artrit (RA) olan 33 hastanın 66 gözü (grup 1), seropozitif RA olan 25 hastanın 50 gözü (grup 2) ve seronegatif ankilozan spondilitli (AS) 26 hastanın 52 gözü (grup 3) çalışma kapsamına alındı. Otuz sağlıklı bireyin 60 gözü de kontrol grubu olarak değerlendirildi. Bütün olgulara ön segment OKT ile santral kornea kalınlığı (SKK) ve temporalden periferik kornea kalınlığı (TPKK) ölçümü yapıldı. Sonuçlar gruplar arasında karşılaştırıldı. Bulgular: Yaş ortalaması grup 1’de 56,30±7,9 yıl, grup 2’de 53,66±9,22 yıl, grup 3’te 43,73±10,86 yıl, kontrol grubunda 51±7,82 yıl idi. Grup 1’de ortalama SKK 518,53 ±31,61μm, ortalama TPKK 704,87 ±46,81 μm idi. Grup 2’de ortalama SKK 536,33±35,12μm iken, ortalama TPKK 723,47±54,57 μm idi. Grup 3’te ortalama SKK 539,52±28,14 μm, ortalama TPKK 742,46±44,45 μm olarak ölçüldü. Kontrol grubunda ise ortalama SKK ve TPKK sırasıyla 540,59±38,94μm ve 740,64±42,28 μm bulundu. Grup 1’de ortalama SKK ve TPKK’ nın kontrol grubuna göre daha ince olması istatistiksel olarak anlamlı idi (p<0,05). Grup 2 ve grup 3’te ortalama SKK ve TPKK ölçümleri kontrol grubuyla karşılaştırıldığında ise istatistiksel olarak anlamlı bulunmadı (p=1,000). Sonuç: Çalışmamızda seronegatif RA hastalarında santral ve periferik kornea kalınlığı, seropozitif RA ve seronegatif AS hastalarına göre daha ince bulundu. Eklem dışı tutulumda önemli bir yere sahip olan RF’nin kornea kalınlığı ile ilişkisi bulunamadı. Anahtar Kelimeler: Seronegatif, Seropozitif, Romatolojik hastalık, Santral kornea kalınlığı, Periferik kornea kalınlığı Central and Peripheric Corneal Thicknesses in Seronegative and Seropositive Rheumatologic Diseases SUMMARY Aim: The aim of this study was to evaluate the central and peripheric corneal thicknesses with anterior segment optical coherance tomography (OCT) in seronegative (rheumatoid factor (RF) negative) and seropositive (RF positive) rheumotogic diseases. Material and Method: Sixty-six eyes (group 1) of 33 seronegative rheumatoid artritis (RA) patients, 50 eyes (group 2) of 25 patients with seropositive RA, and 52 eyes of 26 patients with seronegative ankylosing spondylitis (AS) were composed in this study. Sixty eyes of 30 healthy subjects were evaluated as the control group. All subjects were performed central corneal thickness (CCT) and temporal peripheric corneal thickness (TPCT) measurements with the anterior segment OCT. The results were compared between groups. Results: The mean age was 56.30 ±7.9 years in group 1, 53.66±9.22 years in group 2, 43.73 ±10.86 years in group 3 and 51±7,82 years in the control group. In group 1 the mean CCT was 518.53 ±31.61μm, and the mean TPCT was 704.87 ±46.81 μm. In group 2 the mean CCT was 536.33 ±35.12μm, while the mean TPCT was 723.47 ±54.57 μm. In group 3 the mean CCT was measured as 539.52 ±28.14 μm, and the mean TPCT was measured as 742.46 ±44.45 μm. In the control group the mean CCT and TPCT was 540.59 ±38.94μm and 740.64 ±42.28 μm respectively. The mean CCT and TPCT in group 1 were statistically significantly thinner than the control group (p<0.05). The mean CCT and TPCT in group 2 and group 3 were not statistically significantly different from the control group (p=1.000). Conclusion: In our study it was found that, the central and peripheral corneal thicknesses were thinner in seronegative RA patients when compared to seropositive RA and seronegative AS patients. The RF which has an important role in the extraarticular involvement was not found to be associated with the corneal thickness.
