HLA-B27 İle İlişkili Üveiti Olan Hastalarda Seyir ve Sistemik Tutulum Sevim KAVUNCU, Pınar ÇAKAR ÖZDAL, Ömür DEMİRTAŞ ERSOY, Emine MALKOÇ ŞEN, Faruk ÖZTÜRK ÖZET Amaç: Ön üveit nedeniyle takip edilen HLA-B27 pozitif üveit hastalarında klinik seyrin ve beraberinde ortaya çıkan sistemik hastalıkların incelenmesi. Gereç ve Yöntem: Retrospektif olan bu çalışmada Ulucanlar Göz Hastanesi Uvea Biriminde izlenen 1120 üveit hastası içinden, akut ön üveit nedeniyle başvurup, HLA-B27 antijeni pozitif saptanan hasta dosyaları incelendi. Çalışmaya 62 (%62,5) erkek, 37 (%37,5) kadın olmak üzere en az altı ay takibi olan, 99 hastanın 165 gözü dahil edildi. Dosyalardan; üveitin başlangıç yaşı, hastanın cinsiyeti, göz muayene bulguları, sistemik hastalıklar, üveite ikincil olarak ortaya çıkan komplikasyonlar ve tedaviler kaydedildi. Bulgular: Tanı konulduğundaki yaş ortalamaları; 43,4±12,4 (19-76) yıl, ortalama takip süresi; 36,85 ±44,8 (6-230) ay idi. Hastaların 66’sında (%66,5) tek taraflı, 33’ünde (%33,5) iki taraflı tutulum izlendi. Takip süresince 10 gözde (%6) hipopyon, 22 gözde (%13) katarakt, 9 gözde(%5,5) kistik maküler ödem, 7 gözde (% 4) glokom, 34 gözde (%20) arka sineşi vardı. Son muayenede hastaların 37’si (%37) ankilozan spondilit (AS) nedeniyle sistemik tedavi almaktaydı. Sonuç: Sık tekrarlayan ciddi ön üveit ataklarına rağmen HLA-B27 ile ilişkili üveit hastalarının çoğunda eşlik eden sistemik hastalık yoktur. Ankilozan spondilit, HLA-B27 pozitif ön üveitlere en sık eşlik eden romatolojik hastalıktır. Anahtar Kelimeler: HLA-B27, Ön üveit, Ankilozan spondilit, Komplikasyonlar Clinical Course and Systemic Involvement in Patients with HLA B27- Associated Uveitis SUMMARY Aim: To investigate clinical course and systemic involvement in patients with HLA-B27 associated uveitis. Material and Method: The medical records of 99 HLA-B27 associated uveitis patients followed at Uveitis Department of Ulucanlar Eye Research and Training Hospital were retrostectively evaluated. A total of 165 eyes of 99 patients, (62 men, 37 women), with minimum six months follow-up, were included into the study. The age and gender of patients, follow-up period, systemic diseases, ocular findings, complications related with uveitis and given medical treatment were evaluated from medical records. Results: The mean age at time of uveitis diagnosis was 43.4±12.4 (19-76) year. The mean follow-up time was 36.85±44.8(6-230) months. Ocular involvement was unilateral in 66 patients (66.5%) and bilateral in 33 (33.5%) patients. Hypopion was observed in 10 eyes (6 %), cataracts in 22 eyes (13%), cystoid macular edema in 9 eyes (5.5%), intraocular pressure rise in 7 eyes(4% ) and posterior synechia in 34 eyes (20%) during the follow-up period. Thirtyseven of the all patients had a diagnosis of ankylosing spondylitis (AS). Conclusion: Despite the frequent and severe ocular inflammatory attacks, the long-term visual prognosis of HLA-B27-associated acute anterior uveitis is quite good. Ankylosing spondlitis is the most frequently diagnosed rheumatologic disease in patients with HLA-B 27 positive uveitis. Key Words: HLA-B27, Anterior uveitis, Ankylosing spondilitis, Complications
