Mevsimsel Allerjik Konjunktivitli Hastalarda %0,1’lik Olopatadin Tedavisi ile %0,025’lik Ketotifen Tedavisinin Klinik Sonuçlarının Karşılaştırılması Tuba ÇELİK, Mustafa KÖŞKER, Seda DURGUT EMEÇ ÖZET Amaç: Mevsimsel allerjik konjunktivit (MAK) yönetiminde %0,1’lik olopatadin ile %0,025’ lik ketotifen tedavilerinin etkinliğinin karşılaştırılması. Gereç ve Yöntem: Bu çok merkezli, plasebo kontrollü, randomize prospektif çalışmada MAK belirti ve bulguları olan 124 hasta değerlendirildi. 1. gruptaki (62 hasta) hastaların tek gözlerine %0,1 olopatadin damla damlatılırken diğer gözlerine plasebo uygulandı. 2. gruptaki (62 hasta) hastaların ise tek gözlerine %0,025 ketotifen damla, diğer gözlerine plasebo uygulandı. Belirti ve bulgular, hastalar sorgulanarak ve muayene bulguları kaydedilerek tedavi öncesi, tedavi sonrası 1. günde, 7. günde ve 21. günde 10 puan skalası kullanılarak değerlendirildi. Bulgular: Tedavinin 7. gününde (kısa dönem) %0,1 olopatadin ile kaşıntı yaklaşık %13, kızarıklık %15, yanma %10, yaşarma %11, kemozis %16, mukus sekresyonu %3, kapak ödemi %3 azalırken; ketotifenle kaşıntı %8, kızarıklık %5, yanma %16, yaşarma %19, kemozis %3, mukus sekresyonu %2 ve kapak ödemi %4 azalmıştır. Tedavinin 21. gününde (uzun dönem) olopatadinle kaşıntı yaklaşık %43, kızarıklık %41, yanma %37, yaşarma %38, kemozis %22, mukus sekresyonu %11, kapak ödemi %11 azalırken; ketotifenle kaşıntı %42, kızarıklık %40 ,yanma %46, yaşarma %45, kemozis %23, mukus sekresyonu %10 ve kapak ödemi %12 azalmıştır. Sonuç: Kısa dönemde kaşıntı, kızarıklık ve kemozisi azaltmada olopatadinin daha etkin olduğu görülmüştür. Yanma ve yaşarma belirtilerini iyileştirmede ketotifenin kısa ve uzun dönemde daha etkin olduğu saptanmıştır. Mukus sekresyonu ve kapak ödemini azaltmada ise iki ilaç arasında anlamlı farklılık saptanmamıştır. Anahtar Kelimeler: Allerjik konjunktivit, Olopatadin, Ketotifen Comparison of Olopatadine 0,1% and Ketotifen 0,025% Treatment in Patients with Seasonal Allergic Conjunctivitis SUMMARY Aim: To evaluate the therapeutic effects of olopatadine 0,1% and ketotifen 0,025% in seasonal allergic conjunctivitis (SAC). Material and Method: In this randomised, placebo- controlled, multiple-center study 124 patients with SAC were conducted.The patients were assigned into two groups to receive either olopatadine or ketotifen one eye and placebo in the contrlateral eye. The results were evaluated by summing up the scores using 10 point scale, before, on the first day, the 7th day and the 21th of the treatment. Results: Olopatadine reduced itching 13%, redness 15%, burning 10%, tearing 11%, chemosis 3%, mucous secretion 3%,eyelid eudema 3%; ketotifen reduced itching 8%, redness 5%, burning 16%, tearing 19%, chemosis 3%, mucous secretion 2%, eyelid eudema 3% on the 7th day of the treatment. Olopatadine reduced itching 43%, redness 41%, burning 37%, tearing 38%, chemosis 22%, mucous secretion 11%, eyelid eudema 11%; ketotifen reduced itching 42%, redness 40%, burning 46%, tearing 45%, chemosis 23%, mucous secretion 10%, eyelid eudema 12% on the 21th day of the treatment. Conclusions: Olopatadine was found more effective in relieving itching, redness and chemosis in short period, however ketotifen was superior in reducing burning and tearing both short and long periods. There wasn't significant difference between the two drugs in reducing mucous secretion and eyelid eudema. Key Words: Allergic conjunctivitis, Olopatadine, Ketotifen
