Penetran Keratoplastide Greft Çapının Prognoza Etkisi Züleyha YALNIZ AKKAYA, Ayşe BURCU, Elif ERASLAN YUSUFOĞLU, Mustafa ONAT, Firdevs ÖRNEK ÖZET Amaç: Greft çapının penetran keratoplasti prognozuna olan etkisini incelemek. Gereç ve Yöntem: Greft çapı 8,00 mm ve üzeri olan 21 hastanın 21 gözü (Grup 1) ile greft çapı 8,00 mm’nin altı olan olgulardan rasgele seçilmiş 24 hastanın 24 gözü (Grup 2) retrospektif olarak değerlendirildi. Bulgular: Yaş ortalaması Grup 1’de 46,95±17,4 yaş, Grup 2’de 33,7±15,5 yaş idi (p=0,01). Ortanca takip süresi Grup 1’de 19 (6-120) ay, Grup 2’de 21 (10-96) ay idi (p=0.864). Ortanca alıcı yatak çapı Grup 1’de 8,00 mm (8,00-8,50) ve Grup 2’de 7,50 mm (6,75-7,75) idi. Grup 1’de mükerrer penetran keratoplasti (%61,9), Grup 2’de keratokonus (%58,3) ilk sırada yer alan endikasyonlardı. Takip süresi sonunda Grup 1’de %71,4, Grup 2’de %83,3 gözde greft saydamdı ve iki grup arasında istatistiksel olarak anlamlı fark olmadığı görüldü (p=0,476). Greftin ortalama saydam kalma süresi Grup 1’de 69,8 ay (%95 GA: 40,6-98,99 ay), Grup 2’de 69,8 (%95 GA:49.1-90.5 ay) aydı. Greft çapının greftin saydam kalma süresi üzerine bir etkisinin olmadığı görüldü (p=0,354). Ameliyat sonrası glokom (p=0,203) ve ret reaksiyonu (p=0.889) gelişimi bakımından iki grup arasında anlamlı fark görülmedi. Sonuç: 8,00-8,50 mm’lik alıcı yatak genişliği greft sağ kalımını etkilemediği görüldü. Material and Method: Twenty-one eyes of 21 patients with a graft size of 8.00 mm or larger (Group 1) and 24 randomly selected eyes of 24 patients with a graft size of less than 8.00 mm (Group 2) were reviewed retrospectively. Results: The mean age was 46.95 ± 17.4 years in Group 1 and 33.7±15.5 years in Group 2 (p=0.01). The median follow-up time was 19 (6-120) months in Group 1 and 21 (10-96) months in Group 2 (p=0.864). The median recipient bed was 8.00 mm (8.00-8.50) in Group 1 and 7.50 mm (6.75-7.75) in Group 2. In Group 1 repeated penetrating keratoplasty (61.9%) and in Group 2 keratoconus (58.3%) were the most common indications. At the end of the follow-up, 71.4% of the grafts in the Group 1 and 83.3% in the Group 2 were clear and there was no statistically significant difference between the groups (p=0.476). The mean graft survival time was 69.8 months [(95% confidence interval (CI): 40.6-98.9] in Group 1 and 69.8 months (95% CI: 49.1-90.5) in Group 2. The graft size was not effective on the graft survival time (p=0.35). The groups were similar regarding the postoperative glaucoma (p=0.203) and graft reaction (p=0.889) rates. Conclusion: Recipient bed of 8.00-8.50 mm size is not effective on the graft survival. Key Words: Penetrating keratoplasty, Graft clarity, Graft reaction
