Latanoprost Tedavisi Alan Primer Açık Açılı Glokom Olgularında Travoprost ya da Bimatoprost Monoterapisine Geçişle Görme Alanı ve Santral Kornea Kalınlığı Üzerine Oluşan Etkinliğin Değerlendirilmesi Yusuf KOÇLUK, Kıvanç GÜNGÖR, Oğuzhan SAYGILI, Necdet A. BEKİR ÖZET Amaç: Bu çalışmamızın amacı, latanoprost tedavisi alan primer açık açılı glokom (PAAG) hastalarında, bu ilacı kesip prostoglandin olan travoprost ya da prostamid olan bimatoprosta geçişle oluşan görme alanı ve kornea kalınlığı gibi değişiklikleri incelemek. Gereç ve Yöntem: Mart 2009 - Mart 2010 tarihleri arasında Gaziantep Üniversitesi Tıp Fa kültesi Göz Hastalıkları Ana Bilim Dalı’nda topikal latanoprost monoterapisi ile takipte olan 40 primer açık açılı glokom hastası alındı. 40 PAAG olgusunun latanoprost tedavisi kesildi. Hastaların 20’sinde bimatoprost, 20’sinde ise travoprost monoterapisi şeklinde ilaç değişimi yapıldı. Hastalar 6 ay süre ile takip edildi. Bulgular: Bimatoprost grubunda başlangıç korneal kalınlık 552,80±28,45 mikron olarak, travoprost grubunda 558,85±24,59 mikron olarak ölçüldü. Altıncı ayın sonunda birinci grupta 551,75±27,89 mikron, ikinci grupta 557.25±23.34 mikron olarak bulundu. Bimatoprost kullanan hastalarda başlangıçta ortalama mean deviyasyon (MD) 3,63±3,66 dB, altıncı ayın sonunda ise 3,64±3,59 dB olarak bulundu. Travoprost kullanan grupta ise başlangıçta ortalama MD 4,16±4,49 dB, altıncı ayın sonunda 4,11±4,18 dB olarak bulundu. Sonuç: Sonuç olarak iki grupta da altıncı ay kornea kalınlığında başlangıca göre çok küçük düzeyde bir azalma görüldü. Fakat bu azalma her iki grupta da istatistiki olarak anlamlı bulunmadı. Her iki grupta da başlangıç ve 6. ay ortalama MD değerleri arasında oluşan fark istatistiksel olarak anlamlı değildi. Anahtar Kelimeler: Bimatoprost, Travoprost, Primer açık açılı glokom Evaluation of the Effectiveness on Central Corneal Thickness and Visual Field of Monotherapy with Either Bimatoprost or Travoprost in Patients Who After Latanoprost Therapy in Primary Open Angle Glaucoma SUMMARY Aim: This prospective study aimed to evaluate the efficacy of monotherapy with either bimatoprost or travoprost in patients who after tanoprost therapy in primary open angle glaucoma (POAG). Material and Method: Between March 2009 and March 2010, 40 patients with POAG receiving therapy with topical latanoprost at Gaziantep University Hospital, Department of Ophthalmology were chosen to participate in this study. Latanoprost therapy was stopped in all patients, 20 were chosen to receive monotherapy with bimatoprost and 20 were chosen to receive monotherapy with travoprost. Patients have observed for 6 months. Results: The initial corneal thickness was measured 552.80±28.45 microns in bimatoprost group, 558.85±24.59 microns in travoprost group. At the end of the sixth month, ıt was found 551,75±27,89 microns in the first group and 557.25±23.34 microns in the second group. Average MD was found to be 3.63±3.66 dB at the beginning, 3.64±3.59 dB at the end of the sixth month in patients treated with bimatoprost. Average MD was found to be 4.16±4.49 dB at the beginning, 4.11±4.18 dB at the end of the sixth month in group of patients treated with travoprost Conclusion: As a result, reduction in corneal thickness from baseline was too small in two groups at six month. However, this reduction was not statistically significant in both groups. The difference average MD values in visual field at the end of six months was not statistically significant in both groups.
