Topikal Anestezik İlaçlara Bağlı Toksik Keratopati Elif Betül TÜRKOĞLU, Erkan ÇELİK, Gürsoy ALAGÖZ ÖZET Burada topikal anestezik ilaçların kötüye kullanımına bağlı gelişen toksik keratopatili üç olguyu sunduk. Her 3 olgu gözde ağrı ve görmede azalma şikayetiyle başvurdu. Yapılan muayenesinde kornea ülseri, eşlik eden stromada enfiltrasyon ve hipopiyon izlendi. Tedaviye rağmen iyileşme izlenmeyince her üç olguya amniyon zar transplantasyonu yapıldı. Gizlice topikal anestezik damlatmaya devam ettikleri tespit edildi. Psikiyatri konsültasyonu sonucu bir olguya paranoid şizofreni, iki olguya anksiyete bozukluğu tanısı konularak tedaviye başlandı. Daha sonra bir olguda kendiliğinden kornea perforasyonu gelişince tektonik amaçlı keratoplasti yapıldı. Bir olgumuzda ise korneada nefelyon bırakarak iyileşme izlendi. Topikal anestezik ilaçların kötüye kullanımı ciddi göz komplikasyonlarına neden olabilir. Bu nedenle topikal anesteziklerin göze verdiği zararlar hakkında hastalar, özellikle sanayi kesiminde çalışan işçiler, acilde çalışan pratisyen ve işyeri hekimleri, eczane personeli bilgilendirilmeli, hastalara bu ilaçlar reçetesiz satılmamalıdır. Anahtar Kelimeler: Topikal anestezik, Proparakain, Toksik keratopati Toxic Keratopathy Due to Abuse of Topical Anesthetic Drugs SUMMARY Herein, we report toxic keratopathy due to abuse of topical anesthetic on three cases. All three patients presented with eye pain and blurred vision. On examinations, corneal ulcer with stromal infiltrasyones and hypopyon were found. Despite treatment, there was no recovery and for this reason amniotic membrane transplantation was performed in all three cases. We identified that patients continued using of topical anesthetic secretly. On psychiatric consultation one case was diagnosed paranoid schizophrenia, two cases was diagnosed anxiety disorder. Spontaneous corneal perforation developed in one case and tectonic keratoplasty was performed. In one case, recovery was observed with corneal nephelion. Abuse of topical anesthetic drugs can cause serious ocular complications. For this reason; patients, especially the industrial sector workers, general practitioners and occupational physicians working in the emergency, pharmacy staff informed for potential harm to the ocular surface of topical anesthetics.
