Onbeş yıldır Tip 2 diyabeti olan 60 yaşında erkek hasta rutin göz muayenesi için kliniğimize başvurdu. En iyi düzeltilmiş görme keskinliği Snellen eşeli ile sağ gözde tam, sol gözde 0,1 olarak tespit edildi. Her iki gözde ön segment muayenesi doğal, göz tansiyonları normal olarak ölçüldü. Sol gözde ezotropya mevcuttu. Fundus muayenesinde her iki göz retina venleri çevresinde pigmentasyon ve retinokoroidal atrofi alanları izlendi. Flöresein anjiyografide retinokoroidal atrofi alanlarında pencere defektine bağlı hiperflöresans, pigmente alanlarda blokaja sekonder hipoflöresans görüldü. Humphrey görme alanı testinde lezyonlarla uyumlu bölgede relatif ve absolü skotomlar tespit edildi. Geçirilmiş enflamasyon ve travma öyküsü olmayan hastada mevcut bulgular ile pigmente paravenöz retinokoroidal atrofi olduğu düşünüldü.

Sixty-year-old male patient who had type 2 diabetes for 15 years was admitted to the our clinic for a routine eye examination. Best corrected visual acuity was 1.0 and 0.1 (Snellen) at the right and left eyes, respectively. The intraocular pressures were in normal limits and slitlamp evaluation revealed normal anterior segments. There was esotropia at his left eye. Fundoscopic examination showed marked paravenous pigmentary changes and retinochoroidal atrophy in both eyes. Fluorescein angiography showed hyperfluorescence in the areas with RPE atrophy and hypofluorescence in the pigmented areas due to the masking effect of RPE. There were various absolute and relative scotomas in the visual field (Humphrey) in the corresponding paravenous areas. With these clinical findings, the patient who had no inflammation and trauma history was diagnosed as pigmented paravenous retinochoroidal atrophy.
