İki Taraflı Sporadik Aniridi Vakasının Değerlendirilmesi

ÖZET
Bu çalışmada amacımız glokomu olmayan iki taraflı lens sublüksasyonu ve glukoz tolerans bozukluğunun eşlik ettiği bir sporadik aniridi vakasını sunmaktır.
Yirmi dört yaşındaki erkek hasta, çocukluğundan beri her iki gözde görme azlığı ve fotofobi şikayetleriyle kliniğimize başvurdu. Göz muayenesinde iki taraflı total aniridi, lenslerde subluksasyon, koryoretinal atrofi ve foveal hipoplazi tespit edildi. Sağ gözde görme keskinliği -1,00 (-2,00x1450) tashihle 0,4, sol göz görme keskinliği -2,75x1300 tashihle 0,3’dür. Sağ göz içi basıncı ortalaması 21 mmHg, sol göz içi basıncı 22 mmHg olarak bulunmuştur. Sistemik olarak prediyabetik bir durum tespit edilmiştir.
PAX6 geninin ekspresyon derecesi, görme seviyelerinin vakamızda olduğu gibi hem düşük-orta seviyede korunmasına yol açmış hem de glokom gibi diğer oküler patolojilerin oluşumunu engellemiş olabilir. Hayatı tehdit eden Wilm’s tümörü ve diyabet gibi hastalıklar periyodik olarak takip edilmelidir.

Evaluation of Bilateral Sporadic Aniridia Case
ABSTRACT
The aim of this article is to submit a sporadic aniridia case accompanied by bilateral lens subluxation and impaired glucose tolerance without glaucoma. 24-year-old male patient appealed to our clinic with low vision in both eyes and photophobia since childhood. Bilateral total aniridia, lens subluxation, chorioretinal atrophy and foveal hypoplasia were detected in eye examination. The best corrected visual acuity of the right eye is 0.4 with -1.00 (-2,00x1450), left eye is 0.3 with -2.75x1300. Right intraocular mean pressure was found 21 mmHg, left intraocular pressure was found 22 mmHg. Prediabetic state was detected systemically. The degree of PAX6 gene expression may have prevented both visual acuity to low-middle level and the formation of other ocular pathologies like glaucoma as like as our patient. The life-threatening diseases such as Wilm's tumor and diabetes should be followed periodically.
