Travmatik Keratitli Olguların Takip ve Tedavi Sonuçları

ÖZET
Amaç: Çalışmamızda, travma sonrasında enfektif keratit gelişen olguların takip ve tedavi sonuçlarının bildirilmesi amaçlandı.
Gereç ve Yöntem: Bu retrospektif çalışmamızda, hastanemiz göz kliniği kornea birimine travma sonrasında keratit gelişmesi nedeniyle başvuran hastalar çalışmaya alındı. 35 hastanın kartları geriye yönelik incelendi. Travmaya sebep olan etkenler, alınan kültür ve direkt mikroskopi sonuçları, uygulanan tedavi seçenekleri ve tedavi sonrası alınan sonuçlar değerlendirildi.
Bulgular: Yaş ortalaması 49,25±1,49 olan 35 hastada travmaya neden olan etken, 2 (%5,7) hastada toprak, 24 (%68,6) hastada organik (bitkisel), 5 (%14,3) hastada metal, 3 (%8,6) hastada tırnak yada parmak ve 1(%2,9) hastada ise taş gibi etkenlerdi. Hastaların 16’sında (%45,7) fungal, 10’unda (%28,6) bakteryel, 7’sinde (%20) karışık ve 2’sinde (%5,7) ise klinik ya da mikrobiyolojik olarak tanımlanamayan keratit teşhisleri tespit edildi. Olguların 21’i (%60) medikal tedavi ile iyileşme sağlarken, 14 (%40) olguya medikal tedaviye direnç ve progresyon nedeniyle terapötik penetran keratoplasti ameliyatı uygulandı. 1 (%2,9) olguda ise endoftalmi nedeniyle terapötik penetran keratoplasti ile kombine vitreoretinal cerrahi ameliyatı uygulandı. Tüm olgularda keratit tedavi edildi ve anatomik bütünlük sağlandı.
Sonuç: Travma sonrası gelişen keratit hastalarının mikrobiyolojik incelemesinde farklı etkenler tespit edilebilinir. Medikal tedaviye direnç ve hızlı progresyon gösteren durumlarda cerrahi seçenekleri göz ardı etmemek ve enfektif odağın limbusa doğru yayılımını beklemeden cerrahi uygulamak önemlidir.

Follow-Up and Treatment Results of Traumatic Keratitis
ABSTRACT
Objectives: We aimed to report follow-up and treatment outcomes of cases with infective keratitis after trauma.
Material and Method: This study was conducted in Adana Numune Training and Research Hospital at the Cornea unit in Department of Ophthalmology. The patients who developedkeratitis after trauma included in this study. Thirty-five patients' cards were studied retrospectively. Factors causing trauma, results of culture and direct microscopy, applied treatment options and results after treatment were evaluated.
Results: The mean age of 35 cases were 49.25±1.49 years. The causes of trauma were soil in 2 (5.7%), organic (vegetable) in 24(68.6%), metal in 5(14.3%), nail or finger in 3(8.6%), and stone in 1 (2.9%) of patients. The number of cases with fungal keratitis was 16 (45.7%), with bacterial keratitis was 10 (28.6%), mixed infection was 7 (20%) and microbiologically unidentifiable keratitis was 2 (5.7%) respectively. Twenty-one (60%) of the cases achieved medical treatment and 14 (40%) patients underwent therapeutic penetrating keratoplasty due to medical treatment resistance and progression. 1 (2.9%) patients underwent vitreoretinal surgery combined with therapeutic penetrating keratoplasty for endophthalmitis. In all cases, keratitis was treated and favorable anatomic outcomes were achieved.
Conclusion: Different factors can be detected in the microbiological examination of keratitis patients after trauma. In case that medical treatment is ineffective and there is rapid progression, it is important not to overlook the surgical options and to perform surgery without waiting for the infective focus to extend to the limbus.
