Vernal keratokonjonktivit kronik, çoğunlukla bilateral, ataklarla seyreden oküler yüzeyin allerjik inflamasyonudur. Genellikle pubertenin sonlanması ile spontan remisyona giren bir hastalık olmasına rağmen, psödopitoz ve gelişen kornea komplikasyonları nedeniyle görme keskinliğini etkileyebilmektedir. Bu nedenle topikal anti-inflamatuvar tedavinin etkin ve zamanında başlanması önemlidir. Kliniğimize sağ gözde kızarıklık, ağrı, görme keskinliğinde azalma şikayeti ile başvuran hastanın önce sağ, sonra sol gözünde vernal keratokonjonktivite sekonder “kalkan ülser” gelişti. Hastaya topikal anti-inflamatuvar tedavi başlandı ve amniyon membran transplantasyonu yapıldı.“Kalkan ülseri” düzelen hasta topikal siklosporin-A tedavisi altında ve hala takiptedir. Bu olgu sunumunda vernal keratokonjonktivite sekonder gelişen “kalkan ülser” sonrası uygulanan tedavileri ve sonuçlarını tartışmak amaçlanmıştır.

Vernal keratoconjunctivitis is a chronic, mostly bilateral allergic inflammation of the ocular surface which occurs with seasonal attacks. Although a spontaneous remission may occur with pubertal termination, it can affect visual acuity due to pseudoptosis and may cause corneal complications. For this reason, it is important to start a topical anti-inflammatory treatment timely and effectively. The patient who was admitted to our clinic due to the hyperemia, pain and decreased visual acuity in the right eye developed a “shield ulcer” secondary to vernal keratoconjunctivitis in the right and then in the left eye. The patient underwent topical anti-inflammatory therapy and amniotic membrane transplantation. The patient with complete healing of the “shield ulcer” is still under treatment with topical cyclosporin-A and is still being followed. In this case report, we aimed to discuss the treatment and results of “shield ulcer” secondary to vernal keratoconjunctivitis.
