Aponevrotik ve Nörojenik Etyolojinin Bir Arada Bulunduğu Bir Blefaropitozis Olgusu

ÖZET
Sistemik öyküsünde meme karsinomu ve geçirilmiş serebrovasküler olay mevcut olan 47 yaşındaki kadın hasta, bulanık görme yakınması ile kliniğimize başvurduğunda, sağ gözünde aponevrotik pitozis ve bilateral kronik papil ödem tespit edilmesi üzerine, serebrovasküler olay ve kanser öyküsü olması nedeniyle, papil ödem etyolojisini araştırmak üzere nöroloji bölümüne konsulte edildi. Manyetik rezonans görüntüleme incelemesinde kafa içi metastazlar saptanan hastanın ikinci başvurusunda kapak düşüklüğünde artış olduğu ve tabloya Horner Sendromu eklendiği anlaşıldı. Yapılan ayrıntılı sistemik tarama sonucunda, beyin, mediastinum ve kemik metastazı saptanan hastaya, Evre 4 meme karsinomu tanısı ile palyatif tedavi uygulandı ancak hasta bir süre sonra hayatını kaybetti.
Blefaropitozis etyolojisinde yer alan bazı faktörler, hayatı tehdit eden ciddi durumlardır. Bu patolojilerin atlanmaması için tedavi planı yapılmadan önce, hastaların ayrıntılı bir şekilde ve bütün olarak incelenmesi gerekir.

A Blepharoptosis Case with Aponeurotic and Neurogenic Etiologies
ABSTRACT
A 47 year old female patient, who had breast carcinoma and cerebrovascular accident in her systemic story, came to our clinic complaining about her blurred vision. Since right aponeurotic ptosis and bilateral chronic papil edema was found after her examination and she had cerebrovascular accident and carcinoma history, we have referred her to the neurology department for searching of papil edema etiology. An magnetic resonance imaging analysis has revealed intracranial metastatic masses, and at the second visit of the patient, it was concluded that there was an increase in the degree of right ptosis, and that she had Horner Syndrome. After a detailed systemic investigation, multiple metastasis at brain, mediastinum, and bones were found, and palliation therapy was applied hence she had stage 4 breast carcinoma. However, patient has lost her life after a while.
Some aspects included in etiologic factors list of blepharoptosis can be crucial and life threatening. In order for these serious pathologies to not be skipped, patients must be investigated in detail and as a whole, before deciding treatment plans of eyelid pathology.
