Sağ Göz Travması Geçiren Olgunun Sol Gözündeki Klinik Tablo

ÖZET
Sağ gözüne travma hikayesi nedeniyle acil servise başvuran ancak sol göz kapağı altında metalik yabancı cisim saptanan olgumuzun sunulması amaçlandı.
Ellialtı yaşında erkek hasta sağ gözüne odun çarpması şikayetiyle acil servis polikliniğimize başvurdu. Sağ göz üst ve alt kapaklarında hiperemi ve ödem, sağ gözde subkonjonktival hemoraji ve kornea parasantral saat 9 hizasında 1x2 mm’lik korneal epitel defekti mevcuttu. Sol göz üst kapağında temporalde lokalize 1,5x1,5 cm’lik hiperemik alan dışında diğer bulgular doğaldı. İki yönlü direkt kafa grafisinde sol üst göz kapağı temporal sahaya uyan lokalizasyonda metalik cisimle uyumlu olabilecek yabancı cisim izlendi. Orbital bilgisayarlı tomografide sol üst göz kapağında yaklaşık 0,5 mm boyutunda metalik olduğu düşünülen yabancı cisim varlığı doğrulandı. Cerrahi işlemle metalik yabancı cisim sol üst göz kapak temporalinden çıkarıldı. Hastanın 6 aylık izleminde ek şikayeti olmadı.
Bu olgu, travma hikayesi ile başvuran her hastadan detaylı bir hikaye alınması ve muayenenin diğer gözle karşılaştırmalı olarak yapılması gerekliliğini vurgulamak amacıyla sunulmuştur.

Clinic Features in the Left Eye in a Patient with Trauma to the Right Eye
ABSTRACT
We aim to present a case who was admitted to the emergency department with history of trauma to his right eye however a foreign body was detected under his left eyelid.
Fifty six year old, male patient was admitted to emergency department with complaint of wood trauma to his right eye. Hyperemia and edema in upper and lower right eyelids, subconjunctival hemorrhage and corneal epithelial defect at paracentral 9 o’clock were detected in his right eye. The examination of left eye was normal except 1.5x1.5 hyperemic region at temporal region of upper eyelid. The appearance compatible with the metallic foreign body was determined at X-rays at temporal region of upper left eyelid. The existence of foreign body 0.5 mm in diameter was confirmed by orbital computed tomography. The metallic foreign body 0.6 mm in diameter was extracted from temporal part of upper left eyelid by surgical procedure. There wasn’t any complaints of the patient during 6 months of follow up period.
This case was presented to emphasize the necessity of taking detailed history from the patients who has history of trauma and the examination should be done as compared to the other eye.
