Blefarokonjonktivit Tedavisinde Loteprednol (%0,5)/ Tobramisin (%0,3) Kombinasyonu ile Deksametazon (%0,1)/Netilmisin (%0,3) Kombinasyonunun Güvenlik ve Etkinliğinin Karşılaştırılması

ÖZET
Amaç: Loteprednol (%0,5)/Tobramisin (%0,3) kombinasyonu ile Deksametazon (%0,1)/Netilmisin (%0,3) kombinasyonunun blefarokonjonktivit tedavisinde güvenlik ve etkinliğinin karşılaştırılması.
Gereç ve Yöntem: Bu retrospektif çalışmaya bilateral blefarokonjonktivit tanısı ile tedavi edilen 28 hastanın 56 gözü dahil edildi. Hastalar randomize olarak 14 hasta (28 göz) içeren iki gruba ayrıldı ve bir ay süre ile 4x1 loteprednol/tobramisin kombinasyonu (Grup 1) veya deksametazon/netilmisin kombinasyonu (Grup 2) ile tedavi edildi. Tedavinin etkinliğini araştırmak için, sırasıyla hastaların ve araştırmacıların ilk muayene ve birinci ayda, semptom (ağrı, yanma, kaşıntı, yabancı cisim hissi, ışık hassasiyeti ve bulanık görme) ve bulguları (blefarit, konjonktivit ve kornea tutulumu) değerlendirerek yaptıkları şiddet skorlamalarındaki değişiklikler incelendi. Tedavi güvenliğinin değerlendirilmesinde kombinasyonların yan etki profili ve birinci hafta ve birinci ayda elde edilen göz içi basıncı (GİB) değerleri incelendi.
Bulgular: İki grup, demografik özellikler ve ilk muayenede elde edilen semptom ve bulgu şiddeti skorları açısından karşılaştırılabilir idi. Gruplar kendi içinde değerlendirildiğinde, ışık hassasiyeti ve bulanık görme skorları dışındaki semptomlarda ve kornea tutulumu dışındaki bulgularda anlamlı iyileşme olduğu izlendi. Grup 1 ve Grup 2, birinci ayda elde edilen semptom ve bulgu skorları açısından karşılaştırıldığında iki grup arasında anlamlı fark izlenmedi (p>0,05). Semptom ve bulgu skorlarındaki değişimler de iki grup arasında benzer idi. Birinci hafta ve birinci ayda iki grup arasında GİB değişimleri açısından anlamlı fark yoktu (p değerleri sırasıyla 0,06 ve 0,1).
Sonuç: Bu çalışmada, blefarokonjonktivit tedavisinde, loteprednol/tobramisin ve deksametazon/ netilmisin kombinasyonlarının semptom ve bulgular üzerine benzer etkileri olduğu gözlenmiştir.

Comparison of Safety and Efficacy of Loteprednol (%0.5)/ Tobramisin (%0.3) Combination with Dexamethasone (%0.1) / Netilmicin (%0.3) Combination in the Treatment of Blepharoconjuncitivitis
ABSTRACT
Objective: To compare the efficacy and safety of Loteprednol(0.5%)/Tobramycin(0.3%) combination with Dexamethasone (0.1%)/Netilmycin (0.3%) combination in blepharoconjunctivitis treatment.
Material and Method: Fifty-six eyes of 28 patients who were treated with diagnosis of bilateral blepharoconjunctivitis were included in this retrospective study. Patients were randomly divided into two groups with 14 patients (28 eyes) and treated with loteprednol/tobramycin combination (Group 1) or dexamethasone/netilmycin combination (Group 2) 4 times a day for a month. Changes of the severity scores of symptoms (pain, burning, itching, foreign body sensation, photosensitivity and blurry vision) and signs (blepharitis, conjunctivitis and corneal involvement) which were done by patients and investigators at initial visit and first month were examined to investigate the efficacy of treatment.To evaluate safety of treatment, adverse effect profile of combinations and intraocular pressure (IOP) values at first week and first month were examined.
Results: Two groups were comparable in terms of demographic characteristics and severity scores of symptoms and signs at first visit. Ingroup analysis showed that there were significant improvement in symptoms (except photosensitivity and blurred vision) and signs (except corneal involvement) in both groups. When Group 1 and Group 2 were compared in terms of symptom and sign scores at 1st month, there was no significant difference. Changes of symptom and sign scores were also similar between two groups. There was no significant difference of IOP changes at first week and month between two groups (p=0.06 and p=0.1, respectively).
Conclusion: In this study, loteprednol/tobramycin and dexamethasone/netilmycin combinations showed similar effects on symptoms and signs of blepharoconjunctivitis.
