Gebelerde İkinci ve Üçüncü Trimesterde Gözün Ön ve Arka Bölüm Değişikliklerinin Değerlendirilmesi

ÖZET
Amaç: Gebelerde ikinci ve üçüncü trimesterde meydana gelen ön ve arka segment oküler fizyopatolojik değişikliklerinin değerlendirilmesi.
Gereç ve Yöntem: Çalışmaya hastanemiz gebe polikliniğine rutin muayene için başvuran 120 gebenin 240 gözü dahil edildi. Sağlıklı gebelerin ikinci (14-26 haftalar) ve üçüncü (27-41 haftalar) trimesterde santral kornea kalınlığı, keratometri, göz içi basıncı, santral maküla kalınlığı ve retina sinir lifi kalınlığı ölçümleri yapıldı. Sağ gözler grup 1, sol gözler grup 2 olarak kabul edildi. Grup 1 ve grup 2’den 2. ve 3. trimesterde elde edilen tüm ölçümler birbirleriyle karşılaştırıldı. İstatistiksel analizler için IBM SPSS Statistics 22 programı kullanıldı. p<0,05 ise aradaki fark istatistiksel olarak anlamlı kabul edildi.
Bulgular: K değer ortalaması grup 1 ve grup 2’de 2. trimesterde sırasıyla 43,26±1,26 diyoptri; 43,42±1,28 diyoptri iken; 3. trimesterde 43,58±1,26, 43,68±1,28 diyoptri olarak bulundu. Göz içi basıncı ortalaması grup 1 ve grup 2’de 2. trimesterde sırasıyla 14,24±3,07, 14,23±2,99 mmHg iken, 3. trimesterde 13,05±3,21, 12,85±3,0 mmHg olarak ölçüldü. Santral kornea kalınlığı ortalaması grup 1 ve grup 2’de 2. trimesterde sırasıyla 551,34±30,68 mikron; 553,76±31,67mikron iken; 3. trimesterde 556,91±33,40, 558,65±31,74 mikron olarak bulundu. Retina sinir lifi ortalaması grup 1 ve grup 2’de 2. trimesterde sırasıyla 108,65±12,37; 108,93±9,62;3 mikron; 3.trimesterde 105,28±12,31; 105,62±10,82 mikron olarak bulundu. Santral maküla kalınlığı ortalaması grup 1 ve grup 2’de 2. trimesterde sırasıyla 244,29±24,45; 246,60±29,39 mikron; 3. trimesterde 253,85±28,85, 256,63±31,46 mikron olarak bulundu. Gebelerde 3. trimester 2. trimester ile karşılaştırılınca, 3. trimesterde keratometri, santral kornea kalınlığı ve santral maküla kalınlığı değerlerinde görülen artış ile göz içi basıncı ve retina sinir lifi değerlerindeki düşüş her iki grupta istatistiksel olarak anlamlı bulundu (p:0,001 / p<0,01).
Sonuç: Gebelerde 2. trimester ile 3. trimester karşılaştırılınca 3. trimesterde keratometri, santral kornea kalınlığı, santral maküla kalınlığı değerlerinde artış; göz içi basıncı ve retina sinir lifi değerlerinde düşüş izlenmiştir. Gebelerde oftalmolojik değerlendirme yapılırken bu değişimlerin dikkate alınması gerektiğini düşünmekteyiz.

Evaluation of Anterior and Posterior Segment Ocular Changes in the Second and Third Trimesters of Pregnancy
ABSTRACT
Objective: To evaluate the ocular anterior and posterior segment physiopathological changes during the second and third trimester of pregnancy.
Material and Method: This study consists of, 240 eyes of 120 pregnant women who presented to our hospital for routine pregnancy examination were included. In this prospective study; central corneal thickness, keratometry, intraocular pressure, central macular thickness and retinal nerve fiber layer thickness measurements were taken from the healty pregnants both in the second (14-26 weeks) and third trimesters (27-41 weeks). Right eyes were accepted as group 1 and left eyes were accepted as group 2. The results of second and third trimester were compared statistically. IBM SPSS Statistics 22 program was used for statistical analysis. The p value <0.05 was considered statistically significant.
Results: Average keratometry values of group 1 and group 2 in the second trimester were 43.26±1.26 diopters; 43.42±1.28 diopters and group 1 and group 2 in the third trimester were 43.58±1.26; 43.68±1.28 diopters respectively. Average intraocular pressure of group 1 and group 2 in second trimester were 14.24±3.07; 14.23±2.99 mmHg and group 1 and group 2 in third trimester were 13.05±3.21; 12.85±3.0 mmHg respectively. Average central corneal thickness values of group 1 and group 2 in second trimester were 551.34±30.68 microns; 553.76±31.67 microns and group 1 and group 2 in third trimester were 556.91±33.40; 558.65±31.74 microns respectively. Average retinal nerve fiber layer values of group 1 and group 2 in second trimester were 108.65±12.37; 108.93±9.62 microns and group 1 and group 2 in third trimester were 105.28±12.31; 105.62±10.82 microns respectively. Average central macular thickness values of group 1 and group 2 in second trimester were 244.29±24.45; 246.60±29.39 microns and group 1 and group 2 in third trimester were 253.85±28.85; 256.63±31.46 microns respectively. When the results of the second and third trimesters were compared, the increase of central corneal thickness, keratometry, central macular thickness, intraocular pressure and retinal nerve fiber layer in the third trimester were found statistically significant (p:0.001 / p<0.01).
Conclusion: When we compared the values of the second and third trimesters, we found that in the third trimester the values of central corneal thickness, keratometry, central macular thickness increased, intraocular pressure and retinal nerve fiber layer decreased in comparison to second trimester values. We think that during ophthalmologic evaluation in pregnancy its important to consider these alterations.
