ÖZET

Amaç: Preoperatif kornea astigmatizması olan gözlerde, dik eksende yapılan 4.1 ve 5.5 mm'lik saydam kornea kesili fakoemülsifikasyonun astigmatizmayı düzeltici etkisinin incelenmesi.
Gereç ve Yöntem: Preoperatif kornea astigmatizması 0.5-3.75 D arasında değişen 40 göze dik eksende saydam kornea kesili fakoemülsifikasyon uygulandı. Olgular, kesi büyüklüğüne göre 4.1 mm (Grup I, n=20) ve 5.5 mm (Grup II, n=20) olacak şekilde ikiye ayrıldı. Postoperatif 7, 30, 90 ve 180. günlerde kornea astigmatizma değerleri ölçüldü. Astigmatizma değişikliği ortalama keratometrik silindir (OKS), vektör yöntemi (VY) ve polar değer yöntemleri (PDY) ile hesaplandı. Bulunan değerler, istatistiksel olarak grup içi ve gruplar arasında karşılaştırıldı.
Bulgular: Preoperatif astigmatizma Grup I'de 1.13±0.55 (0.5-2.5), Grup II'de ise 1.95±0.83 (1.0-3.75) D idi (p<0.01). Grup I'de astigmatizmadaki değişiklik 7, 30, 90 ve 180. günlerde sırasıyla OKS ile -0.04, -0,16, -0.3 ve -0,35, VY ile 0.57, 0.38, 0.14 ve 0.25, PDY ile -0.69, -0.73, -0.69 ve -0.55 D, Grup II'de ise OKS ile 0.1, -0.5, -0.89 ve -0.93, VY ile 0.74, 0.78, 0.45 ve 0.42, PDY ile -1.29, -1.06, -1.35 ve -1.08 D idi. Gruplar arasında, 90 ve 180. günlerde astigmatizma değişikliğinde fark vardı (p<0.01). Grup içinde, astigmatizmanın postoperatif değişimi değerlendirildiğinde Grup I' de OKS ve VY ile 90 ve 180. günlerde, PDY ile de tüm dönemlerde, Grup II'de ise OKS ve VY ile 30, 90 ve 180. günlerde, PDY ile de tüm dönemlerde başlangıç değerine göre fark olduğu görüldü (p<0.01).
Sonuç: Fakoemülsifikasyon sırasında, dik eksende yapılan saydam kornea kesisiyle preoperatif astigmatizmayı azaltmak mümkün olup kesi genişliği arttıkça etkisi de artmaktadır.

Lidocain Corneal Astigmatism Correcting Effects of Steep Axis 4.1 and 5.5 mm Clear Corneal Incision Phacoemulsification

SUMMARY

Purpose: To investigate the effects of steep axis 4.1 and 5.5 mm clear corneal incision phacoemulsification on correcting astigmatism in eyes with preoperative corneal astigmatism.
Material and Method: Steep axis 4.1 and 5.5 mm clear corneal incision phacoemulsification was performed in 40 eyes which had 0.5-3.75 D preoperative corneal astigmatism. Patients were divided into two groups according to the incision length. 4.1 and 5.5 mm clear corneal incisions were performed in Group I (n=20) and in Group II (n=20) respectively. Corneal astigmatism was measured 7, 30, 90 and 180 days postoperatively. Astigmatism change was calculated by mean keratometric cylinder (MKC), vector method (VM) and polar value method (PVM). The results are compared statistically both in each group and between the groups.
Results: Preoperative astigmatism was 1.13±0.55 (0.5-2.5) D in Group I and 1.95±0.83 (1.0-3.75) D in Group II (p<0.01). Astigmatism change in Group I was as follows at 7, 30, 90 and 180 days respectively; -0.04, -0,16, -0.3 and -0,35 D by MKC, 0.57, 0.38, 0,14 and 0.25 D by VM, -0.69, -0.73, -0.69 and -0.55 D by PVM, and in Group II 0.1, -0.5, -0.89 ve -0.93 D by MKC, 0.74, 0.78, 0.45 and 0.42 D by VM, -1.29, -1.06, -1.35 and -1.08 D by PVM. There was statistically significant difference between groups at 90th and 180th days according to the astigmatism change (p<0.01). When postoperative astigmatism change was evaluated in each group separately, there was significant change according to the preoperative values in Group I by MKC and VM at 90th and 180th days and by PVM at all days and in Group II by MKC and VM at 30th, 90th and 180th days and by PVM at all days (p<0.01).
Conclusion: It is possible to decrease preoperative astigmatism with steep axis clear corneal incision phacoemulsification and longer incisions correct more corneal astigmatism.
