Mitomisin C ile Trabekülektomi

ÖZET

Amaç: Primer konjenital glokomlu olgularda Mitomisin C (MMC) ile trabekülektomi sonuçlarını değerlendirmek.
Gereç ve Yöntem: 0.2 mg/ml MMC ile trabekülektomi yapılan primer konjenital glokomlu 14 olgunun 24 gözü retrospektif olarak değerlendirildi.
Bulgular: Olgular ortalama 25 ay takip edildi. Operasyon öncesi olguların ortalama göz içi basınçları (GİB) 37.29±4.26 mmHg iken operasyon sonrası 1. ay, 3. ay, 6. ay, 9. ay ve 25. ayda sırası ile 10.75±4.24 mmHg, 11.62±2.71 mmHg, 14.50±5.06 mmHg, 13.58±1.66 mmHg, 15.04±2.42 mmHg idi. Operasyon sonrası son kontrolde 23 gözde (%96) 21 mmHg'nın altında GİB elde edildi. 21 gözde (%88) ilaçsız, 1 gözde (%4) tek, 1 gözde (%4) iki topikal antiglokomatöz tedavi ile 21 mmHg'nın altında GİB elde edildi. Operasyon sırasında 1 gözde vitreus kaybı, 2 gözde hafif derecede hifema geliştiği saptandı. Operasyon sonrası 1. ayda yapılan muayenede 2 hifemanın düzelmiş olduğu gözlendi. 1 gözde pupil ektopisi meydana geldiği saptandı. 1 göze (%4) ikinci bir filtran cerrahi gerekti.
Sonuç: Primer konjenital glokomlu olgularda 0.2 mg/ml MMC ile trabekülektomi operasyon sonrası komplikasyonları az ve başarı oranı yüksek cerrahi bir yöntemdir.

Trabeculectomy with Mitomycin C in Congenital Glaucoma Patients

SUMMARY

Purpose: To evaluate the results of trabeculectomy with Mitomycin C (MMC) in primary congenital glaucoma patients.
Materials and Methods: Twentyfour eyes of 14 primary congenital glaucoma patients who received 0.2 mg/ml MMC combined trabeculectomy were assessed retrospectively.
Results: Mean duration of follow up in patients was 25 months. While mean preoperative intraocular pressure (IOP) was 37.29±4.26 mmHg, mean postoperative IOP was 10.75±4.24 mmHg, 11.62±2.71 mmHg, 14.50±5.06 mmHg, 13.58±1.66 mmHg, and 15.04±2.42 mmHg at 1 month, 3 months, 6 months, 9 months, and 25 months of follow-up respectively. An IOP lower than 21 mmHg was obtained postoperatively in 23 eyes (96%). IOP lower than 21 mmHg was obtained without medication in 21 eyes (88%), with one agent in 1 eyes (4%) and two topical antiglaucomatous medication in 1 eye (4%). During operation, vitreous loss was seen in one eye and mild hyphema was observed in 2 eyes. In these two patients hyphema recovered at the end of the first month. Pupillar ectopia developed in one eye and 1 eyes required additional filtration surgery.
Conclusion: Trabeculectomy with 0.2 mg/ml MMC in primary congenital glaucoma patients is a surgical method that has low postoperative complication and high success rate.
