ÖZET

Amaç: Blefaroptozisli hastalarda silikon bantla yapılan frontal askı ameliyatının etkinliğini belirlemek.
Gereç ve Yöntem: Bu çalışma 1997-2004 yılları arasında silikon bantla frontal askı ameliyatı yapılan 55 hastanın (32 erkek, 23 kadın; ortalama yaş: 19.4 yıl) 76 göz kapağını içerdi. Ptozis 42 hastada (%76) doğumsal, 13 hastada (%24) edinseldi. Frontal askı işlemi, 50 hastada (%91) levator kas işlevinin zayıf olması nedeniyle ve Marcus-Gunn sinkinetik ptozisli 5 hastada (%9) levator kas eksizyonundan sonra yapıldı. Silikon bant 7 hastada göz kapağı kıvrım kesisiyle, 48 hastada kıvrım kesisi yapılmaksızın, beşgen biçiminde yerleştirildi. Hedef kapak aralığına ulaşılması ve iki göz kapağı arasında 1mm veya daha az fark olması halinde ameliyat sonucu başarılı olarak kabul edildi. İzlem süresi 3 ile 39 ay arasında değişti (ortalama: 7.3 ay).
Bulgular: Erken postoperatif dönemde, 15 göz kapağında (%20) az düzelme, 2 göz kapağında (%3) aşırı düzelme meydana geldi. Geç dönemde 6 göz kapağında (%8) nüks ptozis oldu. Beş olguda (%7) alında granülom gelişti. Birincil veya ikincil girişim ve askı ayarlamasından sonra 62 gözkapağında (%82) ameliyat sonucu başarılıydı. Oküler yüzey komplikasyonları noktasal keratopati (20 göz, %26), stromal keratit (2 göz, %2) ve kornea epitel defektiydi (1 göz, %1); sadece iki gözde frontal askı gevşetilmesini gerektirdi.
Sonuç: Silikon bantla frontal askı ameliyatı, ptozisli hastalarda etkin ve başarılı bir tedavi yöntemidir. Major komplikasyonları, rezidüel veya nüks ptozis ve alında granüloma oluşumunu içerir ve görece seyrek olarak görülür. Silikon bant diğer alloplastik askı materyallerine iyi bir alternatif olabilir.

Efficacy of Frontalis Sling Surgery with Silicone Rod

SUMMARY

Purpose: To determine the efficacy of the frontalis sling surgery with silicone rod in patients with blepharoptosis.
Materials and Method: This study included 76 eyelids of 55 patients (32 males, 23 females; mean age: 19.4 years) who underwent silicone rod frontalis sling surgery between 1997-2004. Ptosis was congenital in 42 (76%) and acquired in 13 (24%) patients. The frontalis sling procedure was performed because of poor levator muscle function in 50 patients (91%) and after levator muscle resection in 5 patients with Marcus-Gunn synkinetic ptosis (9%). Silicone rod was placed with an eyelid crease incision in 7 patients and without eyelid crease incision in 48 patients, in a pentagonal fashion. The operation was considered successful when the aimed palpebral fissure height was achieved and when the desired alignment was 1 mm or less between the two eyelids. Follow-up ranged between 3 and 39 months (mean 7.3 months).
Results: In the early postoperative period, undercorrection occured in 15 eyelids (%20) and overcorrection in 2 eyelids (%3). In the late period, ptosis recurred in 6 eyelids (%8). Forehead granüloma developed in 5 cases (5%). The result of surgery was successful in 62 eyelids (82%) after primary and/or secondary attempt and sling adjustment. Ocular surface complications were punctate keratopathy (20 eyes, 26%), stromal keratit (2 eyes, 3%) and corneal epithelial defect (1 eye, 1%); it required loosening of the frontalis sling in only two eyes.
Conclusion: Frontalis sling operation with the silicone rod is an effective and successful method in patients with blepharoptosis. Its major complications include residual or recurrent ptosis and formation of forehead granuloma, and occur relatively rare. The study suggests that the silicone rod may be a good alternative to the other alloplastic sling materials.
