ÖZET

Ağrı kesici, antiinflamatuvar ve ateş düşürücü etkisi nedeni ile aspirin en çok kullanılan ilaçtır. Aspirin siklooksijenaz enziminin geri dönüşümsüz inhibitörüdür. Bu yolla platelret agregasyonunun güçlü mediyatörü olan TxA 2'nin oluşmasını önler. Kararsız angina, ani kalp ölümü, AMİ veya inme gibi olayların temelinde plak yırtılması sonrası gelişen trombosit aktivasyonu vardır. Aspirin trombosit agregasyon ve adezyonunu önleyerek trombüs oluşumunu bloke eder. İskemik kalp hastalığı, inme ve periferik damar hastalığı olmak üzere tüm kardiyovasküler hastalıkların ikincil korunmasında aspirin kullanılmaktadır. Aspirin kullanılmasına rağmen akut arteryel trombotik olay gelişimine, aspirin direnci denilmektedir.

SUMMARY

Aspirin Resistance
Aspirin is the most widely consumed drug in medicine due to analgesic, anti-inflammatory and antipyretic effect. Aspirin is the irreversible inhibitor of the cyclooxygenase enzyme. This way it prevents the formation of TxA 2 which is the strong mediator of the platelet aggregation. At the basis of events such as the unstable angina pectoris, sudden cardiac death, acute MI or stroke, is the platelet activation which develops after plaque rupture. Aspirin use is fundamental in the secondary prevention of all cardiovascular disease, stroke and peripheric vascular disease. Aspirin resistance is responsible for the appearence of the acute coronary syndrome even though aspirin had been used.
