ÖZET

Amaç: Vazovagal senkop tanısında eğik masa testi (EMT) sık kullanılan bir yöntemdir. Ancak EMT birçok faktörden etkilenebilmektedir. Bu çalışmada sağ ventrikül dolum hacminin ve bunun posturel değişiminin vazovagal senkoplu olgularda EMT'yi ne yönde etkilediğini incelemeyi amaçladık.
Gereç ve Yöntem: Çalışmaya vazovagal senkoplu, eğik masa testi pozitif olan 40 (yaş ortalaması: 28±6; 24 kadın) ve eğik masa testi negatif olan 20 (yaş ortalaması: 26±5; 13 kadın) hasta alındı. Hastaların doppler ekokardiyografi ile ayakta ve yatar pozisyonda sağ ventrikül dolum hacimleri hesaplandı. Eğik masa testi pozitif olan hastalarla negatif olan hastalar sağ ventrikül dolum hacimlerinin posturel değişimi yönünden incelendi.
Bulgular: Tüm hastaların ayağa kalkma ile sağ ventriküler dolma hacimlerinde azalma izlendi. Eğik masa testi pozitif olan hastalarda dolum hacmi 85±7ml olarak hesaplanırken negatif olan hastalarda bu değer 91±6 ml olarak hesaplandı. Bu değerler istatiksel açıdan anlamlı derecede farklıydı (p=0.008). Yine aynı şekilde eğik masa testi pozitif olan hastalarda ayağa kalkma ile sağ ventrikül dolum hacmindeki azalma miktarı negatif olan gruba göre anlamlı derecede fazlaydı (p<0.001).
Sonuç: Eğik masa testi sonucu birçok faktörden etkilenebilmektedir. Sağ ventrikül dolum hacmindeki posturel değişimin de eğik masa testi sonucunu değiştirebileceği düşünülebilir.

The Postural Changes of Right Ventricular Filling Volume in Patients with Vasovagal Syncope and influance on Head-up Tilt Test

SUMMARY

Aim: To investigate the postural right ventricular filling volume changes in patients with vasovagal syncope and influance of this volume changes on head-up tilt test results.
Material and Method: The study included 60 patients with vasovagal syncope. 40 of 60 patients (group 1, mean age: 28±6; 24 female) had positive head-up tilt test result and the other 20 patients (group 2, mean age: 26±5; 13 female) had negative. We performed transthorasic echocardiography to all patients both on errect posture and at supin position.We measured right ventricular inflow diameter and Velocity Time Integral (VTI) and calculated the right venricular filling volume for both two position. We compared the postural volume changes between these two groups.
Results: In all patients, the right ventricular filling volume was decreased on errect posture. In goup 1, the right ventricular filling volume on errect posture was 85±7 ml and in group 2, it was 91±6 ml It was statistically different between these two groups. (p<0.001)
Conclusion: There are so many factors that can effect head up tilt test results. According the our study the postural right ventricular filling volume change can influance head up tilt test results
