ÖZET

Amaç: Primer pterjiyumların tedavisinde amnion membran greft uygulamasının nükslere olan etkisinin yanı sıra ameliyat sonrası oluşan kozmetik görünümü tespit etmek.
Gereç ve Yöntem: Yaşları 30 ile 54 arasında değişen (ortalama 44.2) primer pterjiyumu olan ve pterjiyum seviyeleri limbusu l ve 3 mm geçen 17 hastanın (9 bayan 8 erkek) l7 gözü çalışmaya dahil edildi. Daha önce cerrahi uygulanmamış olan pterjiyumlar, primer eksize edildi. Oluşan konjonktival yama, -80 derecede saklanmış amnion membran parçası ile episkleradan ve konjonktivadan sütüre edilerek örtüldü. Sonuçlar pterjiyumun nüksü, kozmetik görünüm, konjonktival hiperemi süresi, greftin vaskülarizasyonu ve hasta memnuniyeti açısından değerlendirildi.
Bulgular: Cerrahi sonrası hastalar 3 ay takip edildi. Hastaların hiçbirinde takip süresinde nüks gözlenmedi. Tüm hastalarda greft vaskülarizasyonu vardı ve greft ayrışması gözlenmedi. Konjonktival hiperemi, tüm hastalarda iki haftaya kadar devam etti. Dokuz hastada (%52.9) bu süre dört haftaya kadar ulaştı. Onyedi hastanın l3'ünde (% 76.4) dış görünüm olarak konjonktivanın normal pembemsi görünümünden ayırt edilebilen membrana ait farklı renk mevcut idi. Dokuz hasta (% 52.9) bu görünümden memnuniyetsiz kaldı.
Sonuç: Pterjiyumların primer eksizyonunda amnion membran grefti uygulaması kolay bir yöntemdir. Operasyon süresi kısa ve maliyeti düşüktür. Çalışmamızda takip süresi kısa olduğu için nüks açısından yorum yapmak zordur. Yöntemin nüksler açısından tam değerlendirilmesi uzun süreli takip sonrası yapılacaktır. Primer pterjiyum tedavisinde amnion membran transplantasyonu uygulamasında greft uyumu ve uygulama kolaylığı avantaj sağlarken, kozmetik olarak ameliyat sonrası dönemde hiperemi mevcudiyeti ve greftin dışarıdan normal konjonktivadan ayırt edilebilmesi hasta memnuniyetini azaltabilmektedir.

Cosmetic Appearance after Amniotic Membrane Transplantation in Treatment of Primary Pterygium

SUMMARY

Purpose: To determine the cosmetic appearance of amniotic membrane transplantation in postoperative period of primary pterygium beside the efficacies in recurrence of disease.
Material and Methods: Seventeen eye of 17 patients (9 women, 8 men) aged 30 to 54 (average 44.2) with primary pterygium that passed limbus 1 and 3 mm made up the study population. Patients non-operated in the beginning with primary pterygium were operated. After the pterygium excision, conjunctival patch were covered with amniotic membrane that preserved in -80 °C by suturation from episclera and conjunctiva. The recurrence of pterygium, cosmetic appearance, time of conjunctival hyperemia, vascularisation of graft, and of patient satisfaction were detected.
Results: The follow-up period was 3 months. Recurrence of disease and dissociation of graft didn't occur in any patients. Graft vascularisation was founded in all of patients. Conjunctival hyperemia occurred in all of the patients throughout two weeks. In 9 patients (52.9%) time of hyperemia extended to four weeks. Different color of membrane was observed in thirteen of seventeen patients (76.4%) from the outside. Nine of these patients (52.9%) didn't be pleased from this condition.
Conclusion: Amniotic membrane transplantation for primary pterygium excision is an easy method. Time of operation is short and cost of procedure is low. In our study we could not make a comment about the recurrence of disease because of the short follow-up time. After the long follow-up time we can explain the recurrence. Graft adaptation and easy application can be advantages in amniotic membrane transplantation for primary pterygium surgery. But in early postoperative period conjunctival hyperemia and whiteness of membrane that observed from the outside can be disadvantages and can reduce the patients satisfaction.
