Distal Pancreatectomy Combined with Splenectomy for Advanced Stage
Epithelial Ovarian Cancer

OBJECTIVES: The objective of this study is to put forward the indications of Distal Pancreatectomy Combined with Splenectomy (DPCS) as a component of extensive cytoreductive surgery of FIGO (The International Federation of Gynecology and Obstetrics) Stage IV epithelial ovarian cancer (EOC) and to evaluate the early postoperative results.
STUDY DESING: Between December 2001 and April 2003, 113 patients with EOC were applied primary and secondary cytoreductive surgery. Ten of these patients had splenic hilus involvement and were operated by using the DPCS procedure.
RESULTS: In 7 patients primary cytoreduction and in 3 patients secondary cytoreduction was performed. One or more of 20 types of abdominal cytoreductive procedures were used in 10 patients beside DPCS. Optimal cytoreduction (residual tumor size?1 cm) was achieved in all patients. Pancreatic tail involvement was histopathologically seen in 8 patients. In postoperative period, there were 6 complications (60%). No mortality was seen.
CONCLUSION: If optimal cytoreduction is foreseen in patients with splenic hilus involvement of ovarian cancer, distal pancreatectomy should be kept in mind. This procedure can be applied to acceptable morbidity and mortality.
