ÖZET

Amaç: Nötropenik hastalarda belirgin morbidite ile birlikte septisemi riski artmıştır. Bu hastalarda uygun geniş spektrumlu antibiyotik tedavisini sağlamak için, her hastanede etkenlerin gösterilmesi ve bunların antimikrobiyal duyarlılıklarının belirlenmesi gereklidir.
Gereç ve Yöntem: Ocak 2002 ile Aralık 2003 arası kliniğimizde febril nötropeni tedavisi almış hastalar prospektif olarak değerlendirildi. Biz 54 febril nötropenik ataktan 67 (%30.2) mikroorganizma elde ettik.
Bulgular: Hematoloji ünitemizde febril nötropenik çocukların kültürlerinde, gram negatif bakterilerin ön planda olduğu (%61.2) görüldü. Gram pozitif izolatlarda (%34.3) Staphylococcus aureus hakimken (%14.9) bunu coagulase-negative staphylococci (%10.4), A Grubu ß hemolytic streptococci (%6.0) ve Streptococcus pneumoniae (%1.5) izlemekte idi. Escherichia coli (%41.8) gram negatif bakteriler arasında en sık görülen etken oldu. Bunu Klebsiella spp. (%12.0), Proteus (%4.5) ve Pseudomonas aeruginosa (%3.0) izlemekte idi. Candida albicans izolatların %4.5'inde görüldü. Gram pozitifler arasında S. aureus en baskın etkendi. Gram pozitif organizmalarda %86.3 oranında penisilin direnci saptandı. Vankomisin ve metisiline karşı direnç saptanmadı. Gram negatif organizmaların dirençli olduğu antibiyotikler ise penisilin, ampisilin, kloramfenikol ve kotrimoksazol olarak saptandı.
Sonuç: Gram negatif bakteri E. coli hastanemizdeki febril nötropenik çocukların kültürlerinden elde edilen esas mikroorganizmadır.

SUMMARY

Investigations of Febris-İnfection Results in the Neutropenic Patients

Objective: Patients with neutropenia have increased risk of septicemia associated with significant morbidity. To provide the appropriate broad-spectrum antimicrobial cover, documentation of causative agents and their antimicrobial susceptibilities should be established in each hospital.
Material and Methods: Patients who were treated for febrile neutropenic in our clinics between January 2002 and December 2003 were evaluated prospectively. We obtained 67 (30.2%) microorganisms from 54 febrile neutropenic episodes.
Result: In the cultures of febrile neutropenic children, gram-negative bacteria prevalent (61.2%) in our hematology unit. Among the gram-positives (34.3%) Staphylococcus aureus were the predominant bacteria (14.9%) followed by coagulase-negative staphylococci (10.4%), Group A ß hemolytic streptococci (6.0%), and Streptococcus pneumoniae (1.5%). Escherichia coli (41.8%) was the most common species among gram-negative bacteria. It was followed by Klebsiella spp (12.0%), Proteus (4.5%), and Pseudomonas aeruginosa (3.0%). Candida albicans was 4.5% of the isolates. Among gram-positives S. aureus was the most common species. Among gram positive microorganisms; the rate of resistance for penicillin was 86.3%, and the resistance for methicillin and/or vancomycin was not received. Although gram negative microorganisms were resistant to chloramphenicol, penicillin, ampicillin and co-trimoxazole.
Conclusion: Gram-negative bacteria E. coli is the main microorganism isolated from cultures of febrile neutropenic children in our hospital.
