ÖZET

Hipertansif kriz medikal aciller içinde önemli bir yere sahiptir. Acil ünitelerine başvuranların %3’ünün, me­di­kal acillerin ise %27'sinin hipertansif krizler olduğu saptanmıştır. Hipertansif krizin temel belirleyicisi kan basıncının mut­lak değerinden çok kan basıncının yükselme hızıdır.
A­ğır hipertansiyonu tanımlamada hipertansif kriz, öncelikli (urgency) ya da acil (emergency) gibi çok çeşitli terimler kul­lanılmaktadır. Hipertansif acil (emergency); akut ve ilerleyici hedef organ hasarı yüzünden kan basıncının ge­nel­lik­le parenteral ilaçlarla hemen düşürülmesi gereken durumları tanımlar. Hipertansif öncelikli durum (urgency); be­lir­gin kan basıncı artışının gözlendiği ancak ciddi semptomların veya hedef organ hasarının gelişmediği bu nedenle kan basıncının daha yavaş, saatler içinde ve genellikle ağız yolundan verilen ilaçlarla düşürülmesi gerektiği du­rum­la­rı tanımlar.
Bu­rada önemli olan nokta, hipertansif aciller ile öncelikli durumlar arasındaki farklılığın kan basıncı düzeyinden zi­ya­de son organ hasarının varlığına bağlı olmasıdır.

Hypertensive Crises

SUMMARY

Hypertensive crisis have a fundamental part in medical emergencies. Hypertensive emergencies cons­ti­tute 3% of patients who attending to emergency services and 27% of patients who have medical emergencies. Ba­sic determining factor for a hypertensive attack is not the absolute value of blood pressure, is its speed of prog­ress.
For description of serious hypertension urgent or emergent hypertensive conditions are used. Hypertensive e­mer­gency is the condition that parenteral drugs should be used to decrease the blood pressure; therefore end-organ in­jury is minimized. Hypertensive urgency means that there is obvious increase in blood pressure, but there are no se­rious symptoms or no end-organ injury found. So in hypertensive urgency blood pressure should be decreased slow­ly through hours using oral drugs.
Dif­ference between emergent hypertensive condition and urgent hypertensive condition is the presence of end-or­gan damage in hypertensive emergencies, not the absolute value of blood pressure.
