ÖZET
Orbita blow-out kırıklarının cerrahi rekonstrüksiyonunda alloplastik implantlar yaygın olarak kullanılmaktadır. Bu implantlara bağlı komplikasyonlar genellikle cerrahi sonrası erken dönemde görülmektedir. Çalışmada sunulan 37 yaşında erkek hasta, sol gözünde ani gelişen çıkıklık ve çift görme şikayeti ile başvurdu. Olgunun 12 yıl önce başka bir merkezde geçirilmiş orbita taban kırığı rekonstrüksiyonu öyküsü mevcut olup, yapılan oftalmolojik muayenesinde sol gözde ekzoftalmus, hiperglobus, bakış kısıtlılığı ve diplopi saptandı. Bilgisayarlı tomografide sol orbitada bir taban implantı ve etrafında yer tutan kitle lezyonu tespit edildi. Lateral kantotomi ve transkonjonktival yaklaşımla orbitotomi uygulandı ve silikon implant etrafında gelişen kapsül içinde hematom ile karşılaşıldı. Silikon implant çıkartılıp, kapsül yaprakları korunarak hematom temizlendi. Postoperatif dönemde, bulgular tamamen düzeldi. Bir yıllık takip süresince enoftalmus, kapak deformitesi veya tekrarlayan hemoraji izlenmedi.

A Rare Late Postoperative Complication of Blow-Out
Fracture Repairment: Proptosis and Diplopia
SUMMARY
Alloplastic orbital floor implants have been widely used to repair orbital blow-out fractures. Most of the complications of alloplastic orbital implants are seen in the early postoperative period. In this study we evaluated a 37 year-old-man who had been operated elsewhere for orbital floor fracture 12 years ago. He presented with sudden proptosis and diplopia in the left eye. Ophthalmological examination demonstrated proptosis, hyperglobus, and restricted ocular motility in the left eye. Computerized tomography showed a collection around an orbital floor implant. Orbital exploration via a lateral canthotomy and transconjunctival incision showed silicone floor implant with hemorrhagic collection around the implant. The silicone implant was removed and the hematoma was drained with preservation of the capsule. Postoperatively, the signs and symptoms resolved completely and enophtalmus, lid deformity or recurrent hemorrhage was not detected during the follow-up period of one year.
