ÖZET
Çalışmamızda, lokal anestezi altında, karbondioksit laser üst kapak blefaroplasti uygulanan bir olguda gelişen tek taraflı geçici görme bozukluğu ve internal oftalmopleji tablosu sunulmaktadır. Elli altı yaşındaki sağlıklı kadın hastada blefaroplasti uygulanırken sağ gözde nasal yağ yastıkçığının çıkarılması sırasında hastanın ağrı hissini kontol edebilmek için ek anestezi verildi. Ameliyat sonrası, sağ gözünün görmediğini belirten hasta değerlendirildiğinde aynı gözde pupilla ışık refleksi kaybı ve midriasis saptandı. Bulgular ameliyat sonrası 3.5 saat içinde tamamen düzeldi. Blefaroplasti uygulaması sırasında kullanılan lokal anestezik madde kas konisi içine geçerek silier ganglion ve optik sinire etki gösterebilir. Bu geçici ama önemli komplikasyona engel olabilmek için lokal anestezi uygulamasında kontrollü olmamız gerekir.

Transient Vision Impairment During Laser
Blepharoplasty Under Local Anesthesia
SUMMARY
We report a case of transient unilateral visual impairment with internal ophthalmoplegia during laser upper lid blepharoplasty under local anesthesia. A 56 year old, healthy woman underwent blepharoplasty in our clinic. During nasal orbital fat removal at the right eye we had to inject additional anesthetic into the nasal fat pad, for pain control. As soon as after the operation the patient complained that she could not able to see with her right eye. Postoperative examination revealed mydriasis at the same eye. All findings resolved completely 3.5 hours after the operation. Local anesthetic drugs which are used during blepharoplasty operation can enter muscle cone and be effective on the optic nerve and ciliary ganglion. We have to use local anesthetics carefully to prevent this transient but important complication.
