ÖZET
Amaç: Yapılan araştırmalar sonucunda inflamatuar ve immünolojik olaylarda önemli rol oynadığı bilinen mast hücrelerinin pterjium gelişiminde etkili olduğu ortaya konmuştur. Ancak nüks pterjiumlarda mast hücrelerinin ne oranda arttığı araştırılmamıştır. Bu çalışmada histokimyasal yöntemle mast hücrelerinin primer pterjiumlarda ve pterjium nükslerinde ne düzeyde olduğunun gösterilmesi amaçlanmıştır.
Gereç ve Yöntem: Bu amaçla gözlerinde ilk kez pterjium saptanan hastalar ile daha önceden pterjium nedeniyle opere olmuş ve yine aynı gözde nüks eden pterjiumlar çalışmaya alınmıştır. Oniki nüks ve 52 primer pterjium olmak üzere toplam 64 hastanın dokuları histopatolojik olarak incelenmiştir. Doku örneklerinde mast hücreleri ışık mikroskopu ile X 400 büyütme altında beş alan sayılarak ortalamaları alınmıştır.
Bulgular: Primer ve nüks pterjiumlara ait doku örneklerinde stromada kan damarları çevresinde toluidin mavisi boyası ile metakromatik granüller içeren mast hücreleri görüldü. Ortalama mast hücre sayısı primer pterjium grubunda 3.84± 2.45 iken nüks pterjium grubunda 5.08±1.83 olarak bulundu. Nüks pterjium grubunda mast hücre sayısı ortalaması primer pterjium grubuna göre daha fazla olmakla birlikte bu fark istatistiksel olarak anlamlı bulunmadı (p=0.165).
Sonuç: Mast hücrelerinin pterjium patogenezindeki etkisi tam olarak bilinmemekle birlikte enflamasyon ve anjiogenez ile ilişkisi, vazoaktif özellikte mediatörler içermesi nedeniyle patogenezde rolü olduğu düşünülmektedir. Primer pterjium olguları ile nüks pterjium olguları karşılaştırıldığında nüks pterjium olgularındaki mast hücre sayındaki artışın pterjium etyopatogenezinde nüks gelişiminde mast hücrelerinin önemli rolü olabileceğini düşündürmektedir.

Mast Cells in Pterygium and Recurrent Pterygium
SUMMARY
Purpose: Mast cells which have been known to play a remarkable role in inflammatory and immunologic cases, has also been revealed to have an effective role in pterygium development . However, mast cell increase rate in recurrent pterygium has not been investigated. This study aims to show the level of mast cells in primary pterygium and recurrent pterygium by histochemical methods.
Material and Methods: The study population is composed of the patients that are firstly diagnosed with pterygium and the patients who were operated due to pterygium and have recurrent pterygium. A total of 64 patient tissues, 12 recurrent and 52 primary pterygium, were examined as histopathologically. Averages on mast cells sample tisues were obtained through light microscope under X 400 magnification by counting 5 areas .
Results: In primary and recurrent pterygium sample tissues mast cells containing metachromatic granules were seen in stromal blood vessels with toluidin blue staining. The average mast cell number in primary pterygium group was 3.84±2.45, whereas it was 5.08±1.83 in recurrent pterygium group. Although mean number of mast cells in recurrent pterygium group was greater than that of primary pterygium group, the difference was not found as statistically significant. (p= 0.1659)
Conclusion: Although the role of mast cells in the pathogenesis of pterygium is not exactly known, it may be contribute to the pathogenesis due to its close relation with inflammatory and immunologic conditions and containing vasoactive mediators. When primer pterygium cases compared to recurrent pterygium cases, mast cells are considered to have an important role in the development of recurrence in the pterygium ethiopathogenesis due to the increase of mast cells in recurrent pterygium cases.
