ÖZET
 
Amaç: Multifokal göz içi lens (GİL) implante edilen katarakt hastalarının fonksiyonel yeterlilikleri ve hasta memnuniyetini değerlendirmek ve hasta memnuniyetini etkileyen faktörleri incelemek.
Gereç ve Yöntem: Bu retrospektif çalışmada, Array SA40N multifokal GİL implante edilen 32 katarakt hastasının 32 gözünün fonksiyonel durumları modifiye edilmiş VF-14 skoru, uzak ve yakın görme keskinlikleri ile hasta memnuniyeti arasındaki korelasyon değerlendirildi. VF-14  skoru, uzak ve yakın görme keskinlikleri ile hasta memnuniyeti arasındaki korelasyon değerlendirildi. Farklı günlük aktivitelerin hasta memnuniyetine etkisi araştırıldı. Takip süresi 8.2±0.6 ay idi.
Bulgular: Tashihsiz görme keskinliği 0.7±0.1, en iyi görme keskinliği 0.9±0.1 idi. Hastaların tümü Jaeger 4 ve daha iyi yakın görmeler elde ettiler. VF-14 skoru ortalama %85.8±18.2 idi.  Ortalama VF-14 skoru yakın grubu sorularında %82, ara mesafe alt grubunda %98,3, ve uzak mesafe alt grubunda ortalama %94 idi. Hasta memnuniyetini en fazla etkileyen aktiviteler; trafik, sokak veya dükkan tabelalarını okumak (r=0.3, p=0.18),  televizyon izlemek (r=0.32, p=0.11), yemek hazırlamak (r=0.403, p=0.02) idi. Hasta memnuniyet anketinde tüm hastalar memnuniyetini en az 'iyi' olarak belirtirlerken, 5  en yüksek (mükemmel) ile 1 en düşük (kötü) değerleri arasında hesaplama yaptığımızda ortalama memnuniyet 5 üzerinden 4.34 olarak hesaplandı.
Sonuç: Multifokal GİL implante edilen katarakt hastalarında ameliyat memnuniyeti yüksektir. Multifokal GİL, yakın, ara mesafe ve uzak görme keskinliğini ilgilendiren günlük işlerde daha az miktarda fonksiyonel kayıp sağlar. Bu faktörler multifokal GİL implante edilen hastalardaki yüksek hasta memnuniyetinin en önemli sebepleridir.

Modified VF-14 Test Application and Patient Satisfaction with Multifocal Intraocular Lens Implantation

SUMMARY
 
Purpose: To  evaluate the functional condition and patient satisfaction with multifocal intraocular lens (IOL) implantation in cataract cases and to assess the factors effecting the patient satisfaction.
Material and Methods: The functional condition of the 32 eyes of 32 cataract patients with Array SA40N multifocal IOL were evaluated with the modified VF-14 questionnaire. The correlation between the patient satisfaction and the VF-14 score; and  the distance and near visual acuities were evaluated. The relationship of different daily functions and patient satisfaction was assessed. The follow up period was 8.4±0.6 months.
Results: In this retrospective study, average uncorrected visual acuity was 0.7±0.1, the best corrected visual acuity was 0.9±0.1. all the patients achieved at least Jaeger 4 or better near visual acuity. The average VF-14  score was calculated 82% in the near subgroup, 98.3% in the intermediate subgroup, and 94% in the distance subgroup. The activities that effected patient satisfaction were; reading traffic, street or store signs (r=0.3, p=0.18), watching television (r=0.32, p=0.11), cooking (r=0.403, p=0.02). According to the patient questionnaire; the patient satisfaction was at least 'good' or better. We calculated the patient satisfaction on a scale between the highest score of 5 (perfect) and the lowest of 1 (bad); the average patient satisfaction was 4.34 out of 5.
Conclusion: The patient satisfaction is high in cataract patients with multifocal IOL implantation. Multifocal IOL causes less limitation in daily tasks regarding near , intermediate and distance vision; and those factors make important contributions to the high satisfaction level with multifocal IOLs.
