ÖZET
 
Amaç: Topikal anestezinin fako-trabekülektomi ameliyatlarındaki etkinliği ve güvenilirliğinin değerlendirilmesi amaçlandı.
Gereç veYöntem: Topikal anestezi ile fako-trabekülektomi ameliyatı yapılan 48 hastanın damla damlatılması sırasında, ameliyat süresince ve ameliyattan sonra hissettiği ağrı sorgulandı. Hastalardan ameliyat sırasındaki doku manipülasyonlarını hissetmelerini 0-3 arasında, cerrahtan ise ameliyat sırasında hastada istemsiz göz hareketlerinin varlığını 0-4 arasında derecelendirmesi ve hareketin cerrahinin hangi aşamasında en çok olduğunu belirtmesi istendi. Hastalara diğer gözden aynı cerrahiyi geçirmeleri gerekse aynı yöntemle ameliyat olmayı tercih edip etmeyecekleri soruldu.
Bulgular: Anestezik damlatılması sırasında hastaların 38'inde hiç ağrı olmazken, 8'inde hafif, 2'sinde orta düzeyde ağrı oldu. Cerrahi sırasında 19'unda hiç ağrı olmazken, 15'inde hafif, 10'unda orta, 4 olguda ise şiddetli ağrı tespit edildi. Cerrahi sonrasında 21 hasta hiç ağrı tariflemezken, 18'i hafif, 9'u orta düzeyde ağrı bildirdi. Cerrahi sırasında cerrahın göz dokularına dokunmasını 14 hasta hiç hissetmezken, 22 zaman zaman, 6 çoğu zaman, 6 hasta ise sürekli olarak hissetti. Yirmiyedi hastada hiç istemsiz göz hareketi yokken, 13 hastada çok az, 3 hastada bazı durumlarda, 2 hastada çoğu zaman, 3 hastada ise sürekli hareket vardı. Yirmibir olgudan 14'ünde en şiddetli olarak koterizasyon sırasında istemsiz göz hareketi ortaya çıktı. Diğer gözden aynı cerrahiyi geçirmeleri gerekse aynı yöntemle ameliyat olmayı tercih edenlerin sayısı 43, etmeyenlerin ise 5'ti.
Sonuç:  Sonuç olarak topikal anesteziyi fako-trabekülektomide maliyet, zaman, hasta ve hekim memnuniyeti, etkinlik, ve güvenilirlik açısından yeterli olması, ve gerekli hallerde ek enjeksiyon anestezilerinin kolaylıkla uygulanabilmesi nedeniyle önermekteyiz.

Efficacy and Safety of Topical Anesthesia in Phaco-Trabeculectomy

SUMMARY
 
Purpose: To evaluate the efficacy and safety of topical anesthesia in combined phacoemulsification and trabeculectomy surgery.
Material and Methods:  Forty-eight patients undergoing phaco-trabeculectomy under topical anesthesia were asked to evaluate pain at the time of drop instillation, during surgery and postoperatively. Patients were asked to score their awareness of surgical manipulations between 0-3. The surgeon scored the patient's involuntary eye movements during surgery between 0-4 and recorded the stage of surgery in which the eye movements appeared the most. The patients were asked if they would prefer the same method of anesthesia in case of a surgery on their fellow eye.
Results: During drop instillation, 38 patients reported no pain, while 8 reported mild,  and 2 moderate pain. Nineteen patients reported no pain during surgery. There was mild pain in 15, moderate in 10, and severe in 4 patients. Postoperatively, 21 patients described no pain, while 18 described mild, and 9 described moderate pain. Fourteen patients were not aware of the surgical manipulations, but 22 occasionally, 6 often, and 6 constantly felt the manipulations. Involuntary eye movements did not appear in 27 patients, appeared rarely in 13, sometimes in 3, usually in 2, and constantly in 3. Of the 21 patients in which eye movements appeared, cauterization was the stage that it appeared the most in 14 patients. Fourtythree patients reported that they would prefer the same method of anesthesia in case of a surgery on their fellow eye.
Conclusion: We conclude that topical anesthesia in phaco-trabeculectomy is cost-effective, safe, effective, comfortable for both the patient and the surgeon, and also beneficial in that additional methods of anesthesia can be used easily if required.
