Retinoblastomlarda Primer
Enükleasyon Tedavisi

ÖZET
 
Amaç: Primer enükleasyon yapılan evre 5 retinoblastom hastalarının klinik özelliklerini belirlemek
Gereç ve Yöntem: Kasım 1998 ile Şubat 2005 tarihleri arasında primer enükleasyon yapılan 22 hastanın 22 gözü çalışmaya alındı. Hastaların 10'u erkek 12'si kız olup yaş ortalaması 62.3 ay (19-119) idi. Tüm hastaların tam oftalmolojik muayenesi yapıldı ve tümör taban çapı, tümör kalınlığı, subretinal sıvı, subretinal tohum, vitreus tohumu varlığı araştırıldı. Tümörler, Reese Ellsworth (RE) sınıflandırma sistemine göre sınıflandırıldı.
Bulgular: Tüm gözlerde RE evre 5 retinoblastom muvcuttu. Bir hasta dışında tüm hastalarda tek taraflı retinoblastom mevcuttu. Tümör taban çapı ortalama 19.0 mm (11-25) idi. Tümör kalınlığı ortalama 10.6 mm (3-16) idi. Onüç gözde (%59.1) vitreus tohumu, dokuz gözde (%40.9) retina altı sıvısı, 7 gözde (%31.8) retina altı tohum, 4 gözde (%18.2) total retina dekolmanı, 3 gözde (%13.6) neovasküler glokom ve 1 gözde (%4.5) sekonder glokom  mevcuttu. Ortalama izlem süresi 25.9 ay (4-75) idi. Tüm gözlere primer enükleasyon uygulandı ve bilgilerine ulaşılan 17 gözün 3'üne vikril mesh kaplı biyoseramik implant, 12'sine vikril mesh kaplı hidroksiapatit implant uygulandı. İki göze orbital implant uygulanmadı. Ameliyata bağlı herhangi bir komplikasyon gelişmedi. Sklera invazyonu gösteren bir olguda santral sinir sistemi metastazı gelişti ve olgu eks oldu.
Sonuç: Evre 5 tek taraflı retinoblastomlarda enükleasyon ve biyoentegre orbital implant yerleştirilmesi uygun bir primer tedavi yöntemidir.

Primary Enucleation in Unilateral Reese Ellsworth Group V Retinoblastoma

SUMMARY
 
Purpose: To assess the clinical findings of the retinoblastoma patients treated with primary enucleation.
Materials and Method: Twenty two eyes of 22 patients with  retinoblastoma that underwent primary enucleation between November 1998 and February 2005 were included in this study. Ten patients were males and 12 were females. The mean age of the patients was 62.3 months (range: 12-119). All patients underwent complete ophthalmological examination and tumor base diameter, tumor thickness, presence of subretinal fluid, subretinal seeds and vitreous seeds were assessed. The tumors were classified according to the Reese-Ellsworth (RE) classification system.
Results: All eyes had RE group V retinoblastoma. The mean tumor base diameter was 19.0 mm (range: 11.0-25.0mm) and the mean tumor thickness was 10.6 mm (range: 3.0-16.0mm). Vitreous seeds were present in 13 of 22 eyes (59.1%), subretinal seeds in 7 of 22 eyes (31.8%), and subretinal fluid in 9 of 22 eyes (40.1%) at baseline. There were 4 eyes (18.2%)  with total retinal detachment, 3 eyes (13.6%)  with neovascular glaucoma and 1 eye (4.5%) with secondary glaucoma. All eyes underwent primary enucleation and in 17 eyes where information was available, a vicryl mesh-wrapped hydroxyapatite implant was inserted in 12 eyes and a vicryl mesh-wrapped bioseramic implant in 3 eyes. An orbital implant was not used in two eyes. There were no intra or postoperative complications related to surgery. The mean follow-up was 25.9 months (range: 4-75). One case having scleral invasion developed central nervous system metastasis and died.
Conclusion: Primary enucleation and placement of biointegrated implant is an appropriate treatment option in unilateral RE group V retinoblastoma with advanced intraocular disease.
