Primer Alt Oblik Kas Hiperfonksiyonunun Tedavisinde Miyektomi Yönteminin Sonuçları Orhan AYDEMİR, Burak TURGUT ÖZET Amaç: Primer alt oblik kas hiperfonksiyonu bulunan olgulara uygulanan alt oblik kas miyektomi yönteminin sonuçlarını değerlendirmek. Gereç ve Yöntem: Bu retrospektif çalışmaya alt oblik zayıflatma cerrahisi geçirmiş 24 olgu (40 göz) dahil edildi. Alt oblik kas hiperfonksiyonu, 0’dan +4’e kadar derecelendirildi. Olguların 16’sına (%66,6) bilateral, 8’ine (%33,4) unilateral miyektomi uygulandı. Bulgular: Tüm olgular cerrahi sonrası en az 6 ay takip edildi. Postoperatif takiplerde 36 gözde (%90) tam düzelme mevcutken 4 gözde (%10) rezidü alt oblik kas hiperfonksiyonu saptandı. Miyektomi sonrası 4 gözde (%10) yoğun subkonjonktival hemoraji gelişirken hiçbir olguda alt oblik kas hipofonksiyonu ve yapışma sendromu gözlenmedi. Sonuç: Primer alt oblik kas hiperfonksiyonunun tedavisinde uygulanan miyektomi yöntemi etkin ve güvenilir bir yöntemdir fakat ameliyat sonrası gelişebilecek subkonjonktival kanama açısından dikkatli olunmalıdır. Anahtar Kelimeler: Primer alt oblik kas hiperfonksiyonu, Miyektomi Results of the Myectomy in the Treatment of Primary Inferior Oblique Muscle Overaction SUMMARY Aim: To evaluate the results of inferior oblique muscle myectomy that were performed in cases with primary inferior oblique muscle overaction. Material and Method: This retrospective study included 24 patients (40 eyes) who underwent weakening of the inferior oblique muscle. Inferior oblique muscle overaction was graded from 0 to +4 overaction. Unilateral myectomy was performed to 8 patients (33.4%) and bilateral to 16 patients (66.6%). Results: All patients in the study were followed up for at least 6 months. While there were full corection in 36 eyes (90%), residual inferior oblique muscle overaction was observed in 4 eyes (10%). Severe subconjunctival hemorrhage was developed in 4 eyes (10%) after myectomy but adherence syndrom or inferior oblique muscle underaction were not observed in any of the cases. Conclusion: Myectomy of the inferior oblique muscle is safe and effective method for primary inferior oblique muscle overaction but surgeons should be carefull for the postoperative subconjunctival hemorrhage.
