Özet

Amaç: Topikal siklosporin A %0,05 göz damlasının atopik keratokonus hastalarında greft reddini önlemede etkinliğini ölçmek.
Gereç ve Yöntem: Keratokonus tanısı ile penetran keratoplasti (PK) yapılan hastalardan atopik durumları ile ilgili anket doldurmaları istendi. Anketler hastalarda greft reddi hikayesi bilinmeden yorumlandı. Hasta dosyaları greft red reaksiyonu insidansı ve özellikleri ve topikal siklosporin A %0,05 kullanımı yönünden değerlendirildi.
Bulgular: 1 Kasım 2007 ve 28 Şubat 2008 tarihleri arasında ardışık 75 keratokonus hastası çalışmaya davet edildi. Bu hastaların 62’sinde (%82,7) atopik durum tespit edildi. Atopik hastaların 23’ ü (37 göz ) topikal siklosporin A %0,05 kullandığı (tedavi grubu), kalan 39 hastanın (52 göz) ameliyat sonrası yalnızca topikal steroid tedavisi aldığı görüldü (kontrol grubu). En sık karşılaşılan atopik durum allerjik rinit olup tedavi grubu hastalarının 21’inde (%91.3) kontrol grubu hastalarının tamamında 39 hasta (%100) tespit edildi. Tedavi grubunda PK sırasında ortalama yaş anlamlı derecede daha düşük (p=0,008) ve alıcı yatağın 8,25 mm ve üstünde olduğu olgu sayısı daha fazlaydı (p=0,013). Greft reddi insidansı %95 güvenirlilik aralığı ile birlikte tedavi grubunda 10,4 (5,1-21,4), kontrol grubunda 8,5 (5,5-13,0) idi (p=0,59). Tedavi grubunda greft reddi insidansı topikal siklosporin A %0,05 ile tedavi süresince ve aynı hastalarda topikal siklosporin A %0,05 ile tedavi edilmediği sürede sırası ile 10,9 (3,8-31,4) ve 17,5 (10,5-29,4) olarak belirlendi (p=0,42).
Sonuç: Topikal siklosporin A % 0,05, atopik keratokonus hastalarında PK sonrasında greft reddi insidansını düşürmede etkin değildir.

Efficacy of Topical Cyclosporine A 0.05% Treatment in Preventing Graft Rejection in Keratoconus Patients with Atopy

SUMMARY

Aim: To evaluate the efficacy of topical cyclosporine A 0.05% treatment for the prevention of graft rejection in keratoconus patients with self reported atopy.
Material and Method: The patients who underwent penetrating keratoplasty (PK) secondary to keratoconus filled out the questionnaire regarding their personal history of atopic disease. The questionnaires were reviewed without the knowledge of graft rejection. Charts were retrospectively reviewed with regard to the incidence and characteristics of graft rejection and the use of topical cyclosporine A 0.05% eye drops.
Results: From November 1st 2007 to February 8th 2008, 75 consecutive keratoconus patients were enrolled. 62 of 75 patients (82.7%) were atopic.23/62 patients (37 eyes) were treated with topical cyclosporine A 0.05% (the treatment group). The remaining 39 atopic patients (52 eyes) served as a control. The most common type of atopy was allergic rhinitis observed in 21 patients (91.3%) in the treatment group and 39 patients (100%) in the control group. The mean age at the time of surgery was younger (p=0.008) and the number of eyes with host trephination size greater or equal to 8.25 mm was higher in the treatment group (p=0.013). The mean follow-up after PK was 13.8±6.1 years (6.1-26.8) in the treatment group and 12.5± 5.8 years (2.5-26.8) in the control group (p=0.3). The incidence of graft rejection (per 100 person -years) with 95% confidence interval was 10.4 (5.1-21.4) in the treatment group and 8.5 (5.5-13.0) in the control group (p=0.59). In the treatment group, the incidence of graft rejection episodes dring periods of treatment with A 0.05% and the periods when the same patients are not being treated with Restasis© was 10.9 (3.8-31.4) and 17.5 (10.5-29.4), respectively (p=0.42).
Conclusion: Cyclosporine is not effective in reducing the risk of graft rejection in atopic keratoconus patients.
