Herpetik Korneal Endotelit Yonca A. AKOVA, Leyla ERKANLI ÖZET Altmış beş yaşında kadın hasta kliniğimize sol gözde fotofobi, kızarıklık ve görmede azalma şikayetleriyle başvurdu. Oftalmolojik muayenesinde görme keskinliğinin sağ ve sol gözlerde sırasıyla 1,0 ve 0,6 düzeylerinde, göziçi basıncı ölçümlerinin ise sağ gözde 16mmHg, sol gözde 24 mmHg olduğu görüldü. Biyomikroskopik muayenesinde sol gözde, endotelde periferden santrale doğru uzanan horizontal paralel lineer ince keratik presipitatlar, o bölgede lokalize stromal ve epitelyal kornea ödemi, Descemet kırışıklıkları, ön kamarada ise 2 (+) hücre reaksiyonu saptandı. Hastaya herpetik lineer endotelit tanısı konularak, sistemik asiklovir 2x800 mg, topikal asiklovir pomad 5x1 ve topikal prednizolon asetat 8x1 başlandı. Dördüncü haftada keratik presipitatların tamamen kaybolduğu, kornea ödeminin gerilediği ve göz içi basıncının 12mmHg’ya düştüğü görüldü. Anahtar Kelimeler: Herpes simpleks virüs, Lineer endotelit, Herpetik keratoüveit Herpetic Corneal Endothelitis SUMMARY Sixty-five years old female patient admitted to our clinic with the complaints of photophobia, redness and decreased vision in her left eye. Ophthalmological examination revealed best corrected visual acuity of 1.0 and 0.6 for the right and left eyes respectively. Intraocular pressure was 16mmHg OD and 24mmHg OS. On slit lamp examination of the left eye, there was conjunctival hyperemia, horizontal parallel linear non-granulomatous keratic precipitates extending from peripheral cornea to the center with localized stromal and epithelial corneal edema and horizontal Descemet folds. The anterior chamber had a mild cellular reaction. She was diagnosed as herpetic lineer endotheliitis and given acyclovir 800 mg orally 2 times/day, topical acyclovir ointment 5 times/day and topical prednisone acetate 8 times/day. On the fourth week of the treatment there was complete resolution of the keratic precipitates and a visible decrease in corneal edema. The intraocular pressure decreased to 12 mmHg.
