Ortokeratoloji Ö. Ömür UÇAKHAN - GÜNDÜZ ÖZET Ortokeratoloji, yüksek oksijen geçirgenliği olan ters-geometri kontakt lenslerin kullanımı ile kornea şekli değiştirilerek refraktif hataların geri dönüşümlü olarak azaltılması veya giderilmesidir. Miyopi düzeltilmesinde bu kontakt lensler gece takılıp, sabah uyanınca çıkarılarak kornea santralinde bir düzleşme sağlanır ve uyanık olunan gündüz saatlerinde miyopinin düzeltilir. Yaklaşık -4,5 D’nin altında miyopisi olup, 1,50 D’den düşük kurala uygun veya 1,00 D’den düşük kurala aykırı silindirik kusuru olan, mesleki sınırlılıklar veya kuruluk/allerji gibi nedenlerle gün-boyu gözlük veya kontakt lens kullanamayan ancak refraktif cerrahi ile geri dönüşümsüz düzeltme istemeyen kişiler için ortokeratoloji alternatif bir düzeltme yöntemidir. Yöntem, dikkatli seçilmiş hastalarda, tecrübeli personel, çok iyi hasta eğitimi, sıkı bir takip ve hasta uyumluluğunun sürekli kontrolü ile uygulanırsa düşük miyopinin geçici olarak düzeltilmesinde etkili ve güvenilirdir. ANAHTAR KELİMELER Ortokeratoloji, Kontakt lens, Miyopi, Refraksiyon Orthokeratology SUMMARY Orthokeratology is the technique that uses high oxygen permeable reverse geometry rigid contact lenses to reshape the cornea to temporarily reduce or eliminate refractive errors. For the correction of myopia contact lenses are worn at night during sleep and removed upon awakening, and the flattening of the central cornea corrects the refractive error during waking hours. Low myopes (less than -4.50 D to -5.00 D) with a with-the-rule cylinder of less than 1.50 D or an against-the-rule cylinder of less than 1.00 D, particularly those who do not wish to use a correction during daytime due to occupational or allergic/dryness reasons, yet, who also do not want an irreversible refractive correction are candidates for orthokeratology. This type of correction is effective and safe when performed on selected patients by experienced practitioners, together with thorough patient education, frequent follow-up, and continuous monitorization of patient compliance.
