Keratokonus Hastalarında Korneal Kollajen Çapraz Bağlama Tedavisinin Kısa Dönemde Kornea Biyomekaniği Üzerine Etkileri Nurullah ÇAĞIL, Özge SARAÇ, Emine AKÇAY, Berna AKSOY, Nagihan UĞURLU, Murat AYAN ÖZET Amaç: Keratokonus hastalarında korneal kollajen çapraz bağlama (CXL) tedavisinin kısa dönemde santral kornea kalınlığı (SKK) ve kornea biyomekaniği üzerine etkilerini araştırmak. Gereç ve Yöntem: Korneal kollajen çapraz bağlama tedavisi yapılan ardışık 40 progresyon gösteren keratokonus hastasının 40 gözü prospektif olarak yapılan bu çalışmaya dahil edildi. Tedavi öncesi ve tedavi sonrası 1. ve 6. aylarda Sirius topografik görüntüleme sistemi ile SKK ölçümü, oküler cevap analizörü (ORA) ile korneal histerezis (KH), korneal rezistans faktör (KRF) ve kornea düzeltmeli göz içi basıncı (GİBkd) ölçümleri yapıldı. Bulgular: Yaş ortalaması 23,15±5,5 (14-40) yıl olan 16 (%40) kadın, 24 (%60) erkek çalışmaya dahil oldu. Ortalama KH CXL öncesi 7,6±0,2 mmHg idi. CXL sonrası 1. ve 6. aylarda sırası ile 7,7±0,2 ve 7,8±0,2 mmHg idi (p=0,609). Ortalama KRF değerleri CXL öncesi ve sonrasındaki 1. ayda ve 6. ayda sırası ile 6,6±0,2, 6,8±0,3 ve 7,0±0,3 mmHg idi (p=0,268). Ortalama SKK CXL öncesi 445,17±6,6 μm idi. CXL sonrası 1. ayda 395,52±6,3 μm, 6. ayda 417,55±6.1 μm idi (CXL öncesi ve sonrası 1. ay p=0.001, CXL öncesi ve sonrası 6. ay p=0,001, CXL sonrası 1. ay ve 6. ay p=0,001). Ortalama GİBkd CXL öncesi ve CXL sonrası 1. ve 6. aylarda sırasıyla 14,9±0,4, 15.0±0,4 ve 14,6±0,4 mmHg idi (p=0,713). Sonuç: Keratokonus hastalarında uygulanan CXL tedavisinin 6 aylık dönemde kornea kalınlığında incelmeye neden olduğu fakat kornea biyomekaniği üzerine etkisi olmadığı görülmüştür. İleri çalışmalarla keratokonus hastalarında CXL tedavisinin uzun dönem sonuçları araştırılmalıdır. Anahtar Kelimeler: Keratokonus, Korneal kollajen çapraz bağlama, Oküler cevap analizörü, Kornea biyomekaniği The Short Term Effects of Corneal Collagen Crosslinking Treatment on Corneal Biomechanics in Keratoconus Patients SUMMARY Aim: To evaluate the effects of collagen corneal-crosslinking (CXT) treatment in keratoconus patients on central corneal thickness (CCT) and cornea biomechanics. Material and Method: Forty eyes of 40 consecutive progressive keratoconus patients who had CXL treatment were included in this prospective study. The CCT was measured with the Sirius topographic imaging system, and the corneal hysteresis (CH), corneal resistance factor (CRF), and cornea compensated intraocular pressure (IOPcc) were evaluated with the ocular response analyzer (ORA) before and 1 and 6 months after the CXL treatment. Results: Sixteen (40%) female and 24 (60%) male patients with an average age of 23.15±5.5 (14-40) years were included in this study. The average CH before CXL was 7.6±0.2 mmHg. After CXL it was 7.7±0.2 and 7.8±0.2 mmHg at the 1st and 6th months respectively (p=0.609). The average CRF before CXL, 1 month and 6 month after CXL was 6.6±0.2, 6.8±0.3, and 7.0±0.3 mmHg respectively (p=0.268). The average CCT was 445.17±6.6 μm before CXL. One month after CXL the average CCT was 395.52±6.3 μm, and 6 months after CXL it was 395.52±6.3 μm (pre-CXL, post-CXL 1st month p=0.001; pre-CXL, post-CXL 6th month p=0.001; post-CXL 1st-6th month p=0.001). The average IOPcc before CXL, 1 month and 6 month after CXL was 14.9±0.4, 15.0±0.4, and 14.6±0.4 mmHg respectively (p=0.713). Conclusion: CXL treatment performed in keratoconus patients caused a reduction in corneal thickness in 6 months period, while having no affect on the cornea biomechanics. Further studies are needed to evaluate the long term affects of CXL treatment in keratoconus patients.
