Silikon Keratopatisinde Penetran Keratoplasti Sonuçları ve Silikonun Greft Sağkalımına Etkisi Baki KARTAL, Baran KANDEMİR, Nadire ERDOĞAN DİB, Süleyman KUĞU, Turan SET, Berkay AKMAZ, Sadullah KELEŞ, Hasan ALTINKAYNAK, Yusuf ÖZERTÜRK ÖZET Amaç: Silikona bağlı keratopati gelişen hastalarda penetran keratoplasti (PK) sonuçlarını değerlendirmek ve greft yetmezliği ile ilişkili risk faktörlerini belirlemek. Gereç ve Yöntem: Silikon keratopatisi (SK) için PK uygulanan 21 hastanın kayıtları pars plana vitrektomi (PPV) ve PK ile ilgili özellikler, PK sonrası greft sağkalımı, greft yetmezliği açısından silikon ve diğer risk faktörleri ile görme keskinliğinde sağlanan değişim açısından retrospektif olarak değerlendirildi. Bulgular: Yaş ortalaması 41,38±18,82 (5 - 68 yaş) olan 18 erkek (%85,7), 3 kadın (%14,3) hastaya SK için PK yapılmıştı. PPV ile PK arası süre 22,36±13,46 (3-72 ay) idi. PPV’nin en sık travmaya bağlı retina dekolmanı (RD) (%61,9) nedeniyle yapıldığı izlendi. PK sırasında 14 olgu (%66,7) afak, 7 olgu (%33,3) psödofak olup; silikon olguların 12’sinde (%57,1) tamamen, 6’sında (%28,6) kısmen çıkarılmış ve 3’ünde (%14,3) alınmamıştı. PK sonrası takip süresi 25,86±27,45 (5-113 ay) idi. Son muayenede 11 hastada (%52,4) greft saydam iken 10 hastada (%47,6) greft yetmezliği gelişmişti ve silikon varlığı ile greft yetmezliği anlamlı derecede ilişkiliydi (p<0,05). Greft sağkalımı açısından glokom, kornea vaskülarizasyonu, hipotoni anlamlı risk faktörleri iken (p<0,05); red atağı, sineşi, re-PK ve geç PK yapılması ile ilişki saptanmadı (p>0,05). Sonuç: PK öncesinde silikonun alınmasının greft açısından anatomik başarı için bir ön koşul olduğunu düşünmekteyiz. Fonksiyonel başarı ise nakil öncesinde arka segmentin durumu ve sonrasında da greft ile ilgili komplikasyonlara bağlıdır. Anahtar Kelimeler: Silikon yağları, Penetran keratoplasti, Greft yetmezliği Results of Penetrating Keratoplasty in Silicone - Induced Keratopathy and the Effects of Silicone in Graft Survival SUMMARY Aim: To evaluate the results of penetrating keratoplasty (PK) in patients with silicone-induced keratopathy and to determine the risk factors associated with graft failure. Material and Method: The medical records of 21 patients who underwent PK for silicone keratopathy (SK) were retrospectively evaluated in terms of characteristics associated with pars plana vitrectomy (PPV) and PK, graft survival after PK, the change in visual acuity provided with silicone and other risk factors in terms of graft failure. Results: PK for SK was carried out on 18 male (85.7%) and 3 female (14.3%) patients with a mean age of 41.38±18.82 years (5-68 years). The duration between PPV and PK was 22.36±13.46 (3-72 months). It was seen that PPV has been conducted mostly due to retinal detachment (RD) (61.9%) caused by trauma. During PK, 14 cases (66.7%) were phakic, and 7 cases (33.3%) were pseudophakic; in 12 (57.1%) of the cases silicone was removed completely, in 6 (28.6%) of the cases it was removed partially, and in 3 (14.3%) of the cases it was not removed. The duration of follow-up after PK was 25.86±27.45 months (5-113 months). On the last physical examination, while the graft was transparent in 11 patients (52.4%), graft failure had developed in 10 patients (47.6%), and the presence of silicone and graft failure was significantly correlated (p<0.05). While glaucoma, corneal vascularization, hypotonia were significant risk factors for the graft survival (p<0.05), no relation was found associated with rejection attack, synechia, re-PK and late PK (p>0.05). Conclusion: We believe that removal of silicone before PK is a prerequisite for the anatomical success of the graft. Functional success on the other hand is defendant on the condition of the posterior segment prior to transplantation and the complications associated with the graft.
