Sorunsuz Katarakt Cerrahisinden Sonra Gelişen Herpetik Dendritik Keratit: Olgu Sunumu Tülay ALPAR AKÇETİN, Nurhan DİNÇER, Fatma ESİN ÖZDEMİR, Kadir ELTUTAR ÖZET Herpese ait klinik öyküsü olmayan bir olguda sorunsuz katarakt ameliyatından sonra gelişen herpetik dendritik keratitin klinik özellikleri ve tedavisini incelemek. Antiglokomatöz tedavi kullanan 69 yaşında erkek hastanın sağ gözüne sorunsuz fakoemülsifikasyon ve göz içi lens (GİL) implantasyonu yapıldı. Ertesi gün yapılan muayenede sağ gözde görme keskinliği 0,5, göz içi basıncı (GİB) 18 mmHg, ön kamarada hafif bir fibrinöz reaksiyonla birlikte 2+ hücre reaksiyonu mevcuttu. Saat başı verilen topikal kortikosteroid (Predforte coll), topikal antibiyotik (Okacin coll) ve 4x1 midriatik tedavi ile 2. gün kontrolünde ön kamarada fibrinöz reaksiyon kaybolmuştu. Topikal kortikosteroid tedavisi 3. günden sonra 3 saatte bir damla olacak şekilde azaltılarak devam edildi. Operasyondan 7 gün sonra yapılan kontrol muayenesinde kornea alt temporalinde tipik dendritik keratit tespit edildi. Topikal kortikosteroid tedavisi sonlandırılarak topikal asiklovir tedavisi başlandı. Antiviral tedavinin 5. gününde yapılan kontrolünde dendritik keratit sekel bırakmadan tamamen düzeldi. Katarakt operasyonundan sonra herpetik dendritik keratit nadir görülen bir komplikasyondur. Ancak herpese ait klinik öyküsü olmasa da cerrahi travma ve yoğun topikal kortikosteroid kullanımı ya da latanoprost ve beta blokör tedavi nedeniyle katarakt operasyonundan sonra herpetik epitelyal keratitin gelişebileceği akılda tutulmalıdır. Anahtar Kelimeler: Herpetik keratit, Katarakt cerrahisi Dendritic Herpetic Keratitis after Uneventful Cataract Surgery: A Case Report SUMMARY To evaluate the clinical characteristics and treatment of developing dendritic herpetic keratitis after uneventful cataract surgery in a patient with no clinical history of herpes simplex keratitis A 69 year-old male patient with a glaucoma underwent the right eye phacoemulsification and intraocular lens (IOL) implantation. The next day after uneventful surgery visual acuity of the patient was 0.5, intraocular pressure (IOP) was 18 mmHg. He had a mild fibrinous reaction with 2+ anterior chamber cell reaction in the right eye. After the tretment with hourly topical prednisolone asetat 1% (Pred-forte coll) in combination with topical antibiotics (Okacin coll) and 4 times a day mydriatic, the fibrinous reaction was disappeared in the postoperative second day. The topical corticosteroid was given one drop every 3 hours in the postoperative third day. Seven days after the surgery the patient had semptoms of photophobia and redness and a typical dendritic herpetic keratitis were observed in the inferior temporal region of the cornea on slit-lamp examination. The topical corticosteroid was replaced by treatment with topical acyclovir, 5 times a day. On the fifth day of antiviral treatment, dendritic keratitis was completely improved without sequelae. Dendritic herpetic keratitis is a rare complication after cataract surgery. However, in patients even with no past history of herpes simplex keratitis, development of herpetic epithelial keratitis after cataract surgery should be considered due to surgical trauma and intensive use of topical corticosteroids or latanoprost and beta-blocker therapy.
